Methods of physical medicine have played an invaluable role in the therapeutic concept of rheumatic diseases far into the second half of the past century -when drug-based treatments were limited and toxic. Since then, the significance of physical medicine has unfortunately been outshined by the rapid therapeutic success of new Disease Modifying Antirheumatic Drugs (DMARDs) and the biologics (so-called "anti-cytokine therapeutics") in the past 20 years. Nevertheless, physical medicine plays a still central role in the multimodal therapy of rheumatic diseases, and is a key component of therapeutic guidelines [1][2][3].According to current data from the German Collaborative Arthritis Centers for the treatment of rheumatoid arthritis (RA), Abstr Ac tA multimodal treatment of rheumatic diseases without physical medicine is incomplete. However, the mechanisms underlying the beneficial effects of physical medicine are only partially understood. Modern molecular medicine offers new insights into pivotal mechanisms and mediators of the inflammatory process and into interactions between cells of the immune system and bone cells. Investigating the potential influences of different methods of physical medicine on the central regulatory processes of inflammatory rheumatic diseases is of great interest, highly fascinating and opens up new avenues of research for the future. This article focuses on the novel results of molecular physical medicine in rheumatic diseases and osteoporosis. ZUsAMMenfAssUnGEine multimodale Therapie von rheumatischen Erkrankungen ohne physikalisch-medizinische Maßnahmen ist stets inkomplett. Die Wirkungsmechanismen, die der klinischen Verbesserung durch physikalische Maßnahmen zugrunde liegen, sind bisher nur teilweise bekannt. Methoden der molekularen Medizin ermöglichen es inzwischen zentrale Botenstoffe des inflammatorischen Geschehens -die u. a. für die Wechselbeziehungen zwischen Knochen-und Immunzellen von zentraler Bedeutung sind -und deren Beeinflussung durch Applikation physikalischer Therapiemaßnahmen zu analysieren. Der Eingriff in dieses zentrale Regulationssystem bei rheumatischen Erkrankungen und Osteoporose durch Physiotherapeutika ist spannend und faszinierend zugleich und bietet zukünftig ein großes Forschungsfeld. In healthcare the term interface describes the communication and the sharing of responsibilities between different aspects of medical care und the different professional groups in medicine. It enables cooperation without conflicts and can contribute to an improvement of healthcare and reduce healthcare costs. The postgraduate professional education, medical guidelines and therapy recommendations are an important basis for the definition of interfaces. The definition of such an interface between different healthcare groups is essential for the implementation of selective contracts with health insurance companies. An appropriate health care interface between general practitioners and rheumatologists has been defined as well as between hospital and ambulant rheumatology treatment. The division of responsibilities between orthopedists and rheumatologists is still under discussion. A proposal for such an interface from the point of view of rheumatology is presented.

The clinical course of axial spondyloarthritis (SpA) is variable and characterized by chronic back pain and extraspinal manifestations, such as asymmetrical arthritis, dactylitis and enthesitis. Extra-articular manifestations in the eyes (anterior uveitis), skin (psoriasis) and intestines (chronic inflammatory bowel disease) are also frequent manifestations in patients with SpA. Due to the heterogeneity of disease manifestations and the partial concentration on structural alterations in the sacroiliac joints visible in X‑ray images, the diagnosis is often delayed for many years. An important step in the direction of improved early recognition of axial SpA was establishment of the Assessment of SpondyloArthritis International Society (ASAS) classification criteria published in 2009, which focused on the initally deep-seated back pain and chronicity in relatively young patients as well as the importance of magnetic resonance imaging and HLA B 27 determination in the early stages of the disease. In order to achieve the foundations for an effective and timely therapy of affected patients, in 2014 on the initiative of the German Society of Rheumatology, S3 guidelines on axial SpA including Bechterew's disease and early forms were formulated in cooperation with other specialist societies. This article gives an overview of the contents of the S3 guidelines on axial SpA.
