Background: The purpose of this study is to determine if post-operative administration of dexamethasone reduces the risk of postoperative hypocalcemia and low parathyroid hormone levels. Methods: We compared 2 groups of patients (group A and group B). Group A received post operative dexamethasone post operatively and group B didn't receive any glucocorticoids post operatively. Calcium and parathyroid hormone levels were measured pre and post operatively, patients were followed up for 30 days post operatively. Symptomatic hypocalcemia was the primary outcome. Results: 50 patients were included, 25 patients in each group. 4% of group A patients developed symptomatic hypocalcemia while 28% in group B patients developed post operative symptomatic hypocalcemia which was statistically significant at P value of 0.021. Each group had one patient whom had developed persistent hypocalcemia with low parathyroid hormone levels. There was no statistical significance regarding other complications between the 2 groups. Conclusions: This trial demonstrated that the use of low dose dexamethasone post operatively following total thyroidectomy reduced the incidence of post operative symptomatic hypocalcemia with no effect on other complications
