Dexamethasone inhibits endotoxin‐induced coagulopathy in human lungs

Abstract: Essentials Glucocorticoids are associated with an increased risk of thrombosis.Healthy volunteers received dexamethasone or placebo in an endotoxin lung instillation model.Dexamethasone suppressed thrombin generation in bronchoalveolar lavage.Glucocorticoids inhibit endotoxin induced pulmonary coagulopathy. SummaryBackgroundActivation of local and systemic coagulation is a common finding in patients with pneumonia. There is evidence that glucocorticoids have procoagulant activity in the circulation, particula… Show more

“…Dexamethasone has been shown to reduce sepsis-induced coagulation activation in the lungs, among other anti-inflammatory actions. 52 The effect of heparin in limiting progression to ARDS observed in this study was predominantly due to fewer patients developing bilateral pulmonary infiltrates, suggesting heparin might also limit progression of pulmonary oedema. 53 Of particular current interest, the spike protein of SARS-CoV-2 and other associated coronaviruses have been shown to bind to heparan sulphate receptors expressed by respiratory epithelial cells, leading to cellular invasion.…”

“…For subjects enrolled in the open-label study cohorts, bronchoscopic sampling was performed to obtain BAL fluid either on days 1 and 30 or on days 2 and 8, according to the current guidelines (22, 23) and as described previously (24). During bronchoscopy, a total of 100 to 300 ml of prewarmed sterile isotonic saline was instilled and retrieved in three to five aliquots through the bronchoscope from the middle lobe or the lingula.…”

“…GCs failed to attenuate the LPS-induced coagulation cascade in a human model of sepsis (177). In contrast, Bartko and coauthors showed that GCs locally reduce coagulation in a human model of lung inflammation with LPS (181). Different experimental settings used in these studies may in part explain contradictory results.…”
