Device closure of patent ductus arteriosus: optimal guidance by transaortic phased-array imaging

Abstract: In many institutions, device closure of patent ductus arteriosus (PDA) in adult patients is only performed once to twice per year. This report describes an interventional PDA closure procedure being effectively assisted by transaortic phased-array imaging, also known as intracardiac echocardiography (ICE). This way, well-known advantages of ICE as are lowering of radiation exposure and fluoroscopy time can potentially be transferred to PDA device closure.

“…The ICE catheter is positioned in the descending aorta and the probe aimed at the infundibulum of the PDA, depicting continuous shunt flow and monitoring all procedural steps 28 ( Figure 8 ). The advantages of ICE demonstrated for device closure of IAC, including lower radiation exposure and shorter fluoroscopy times, can be potentially transferred to device closure of peri-membranous VSDs and PDAs.…”

“…Though trans‐aortic echocardiography was reported to guide DC‐PDA , TPE has several advantages. Trans‐aortic echocardiography needs to place an 8 French sheath into the femoral artery, and that is undesirable in patients on anti‐coagulation therapy.…”

“…Overcoming this disadvantage of TEE, intra‐cardiac echocardiography (ICE), especially using AcuNav™ (Biosense Webster Inc, Diamond Bar, CA, USA), has been developed and widely used in catheter intervention for inter‐atrial septum, closure of atrial septal defect or perforation of inter‐atrial septum, with local anesthesia . Recently ICE catheter has expanded its application and was used in the other types of catheter interventions including percutaneous pulmonary or aortic valve replacement and balloon valvuloplasty . However, there is no report of the ICE catheter used in the pulmonary artery to guide DC‐PDA.…”
