AbstractIn order to investigate human and organisational factors in offshore accidents, two human factors incident reporting forms: the Open Reporting Form and the Comprehensive Reporting Form, were developed and evaluated. Using statistical techniques, the form with more structured questions was found to provided 66% more information than the participating company's existing reporting form. The results of the research are described focussing on potential benefits for the offshore oil industry.
