Fetal and early postnatal life represent critical periods in vertebrate immune system development. Disruption of such development by perinatal immunotoxic chemical exposure has been widely described in experimental animal models. The resultant inhibited postnatal immune responses in such animals are often more dramatic and persistent than those after exposure during adult life. Further, recent reports suggest that prenatal exposure to immunotoxicants may exacerbate postnatal aberrant immune responses (e.g., hypersensitivity disorders and autoimmune disease) in genetically predisposed rodents. Limited information is available regarding the possibility of inhibited postnatal immune capacity in humans as a result of developmental immunotoxicant exposure. The multifactorial nature of hypersensitivity and autoimmune responses will further complicate the elucidation of possible relationships between chemical exposure during ontogeny of the human immune system and immune-mediated disease later in life. Taken together, however, the available animal data suggest the potential for altered postnatal immune function in humans exposed to immunotoxicants (e.g., environmental chemicals and therapeutic agents) during fetal and/or early postnatal life.

In utero cell and gene therapies constitute alternative strategies to the postnatal treatment of inherited diseases. Fetal hematopoietic progenitors could be a potential source of donor cells for these strategies. In this study, hematopoietic lineage-negative fetal liver cells from 14.5-day-old fetuses were transduced under different cytokine and culture combinations using a lentiviral vector expressing the enhanced green fluorescent protein (EGFP). When cells were transduced for 6 h in the presence of mSCF, hTPO and FLT3-L in retronectin-coated dishes at a multiplicity of infection of 10 transduction units/cell, up to 70% of granulo-macrophage colony-forming cells expressed the EGFP reporter gene. In utero transplantation experiments revealed that conditions leading to high transduction efficiencies were associated with poor engraftments of syngeneic recipients. Significantly, this effect was associated with the detection of a humoral and cellular immunoresponse against the transgenic protein. Moreover, the humoral response against EGFP was detected not only in in utero transplanted recipients but also in the operated mothers, suggesting the maternal origin of the anti-EGFP immunoresponse. These observations reinforce the necessity of carefully studying the potential immunoresponses in future prenatal gene therapy protocols.

INTRODUÇÃOA síndrome de imunodeficiência adquirida (AIDS) é a conseqüência da infecção por um vírus de RNA, da família dos retrovírus, denominado vírus da imunodeficiência humana (HIV) 1 . Esse vírus tem tropismo específico para células que apresentam o antígeno de superfície CD4, cujos principais representantes são linfócitos T auxiliares e células do sistema macrofágico-monocitário 2 . A ligação do HIV com a célula hospedeira ocorre por interação entre uma glicoproteína do envelope viral, a gp120, e a molécula CD4 das células hospedeiras, e propicia a internalização da partícula viral. Uma vez no interior da célula hospedeira, o vírus é descoberto e seu RNA é convertido em DNA, por meio da ação da enzima transcriptase reversa. O DNA viral será, então, incorporado ao DNA do hospedeiro, possibilitando o início da sua replicação 3,4 . Em adultos, a infecção aguda pelo HIV-1 é caracterizada pelo surgimento de uma síndrome mononucleósica. Neste período, que precede a soroconversão, o vírus pode ser detectado no líqui-do cefalorraquidiano (LCR), plasma e células mononucleares do sangue periférico (PBMC) 4 . Na fase de soroconversão, ocorre um rápido declínio da quantidade de vírus livres e intracelulares, sugerindo o surgimento de uma resposta imune capaz de limitar a replicação viral 5 . Após essa fase, os pacientes entram em um período assintomático que pode durar vários anos, caracterizado por baixa replicação viral e por declínio contínuo e gradual do número de linfócitos CD4+. Entretanto, apesar da aparente latência, estudos longitudinais demonstraram que a carga viral total aumenta lenta e inexoravelmente nesse período de estabilidade clínica 6 . A progressão da doença caracteriza-se, então, por diminuição rápida do número de linfócitos CD4+, aumento de viremia plasmática e celular, e por surgimento de síndromes clínicas associadas à imunodeficiência.À semelhança do que ocorre em adultos, crianças infectadas pelo HIV-1 apresentam alterações da função imune, predispondo-as a um maior risco de infecções secundárias. Entretanto, crianças infectadas in utero ou no período perinatal apresentam latência relativamente curta antes do surgimento da fase sintomática 7 . A viremia plasmática, nesses pacientes, é superior à daqueles infectados após os três meses de idade 8 . Clinicamente, as crianças infectados por via vertical tendem a apresentar evolução mais rápida e severa do que aquelas infectadas por outras vias 7 . Esses dados sugerem que as variações no espectro clínico, neste grupo de crianças, refletem o grau de imaturidade do sistema imune no momento da infecção pelo HIV-1.Este artigo revê a ontogenia do sistema imune humano e suas implicações na imunopatogenia da infecção vertical pelo HIV-1 em crianças.
