Background: Poland syndrome is classically described as symbrachydactyly, with hypoplasia of the pectoralis major and other upper thoracic musculoskeletal structures. It is thought to be caused by intrauterine interruption in subclavian arterial flow and often includes breast hypoplasia. Affected vasculature can pose a challenge for reconstruction with free flaps because inflow may not be reliable in this patient population. Methods: We present the rare case of a 28-year-old woman with left-sided Poland syndrome, significant family history of breast cancer, and BRCA1+ mutation who underwent bilateral prophylactic nipple-sparing mastectomies with successful immediate bilateral deep inferior epigastric artery perforator free flap reconstruction. The surgical literature in this clinical scenario is also reviewed. Results: Preoperative computed tomography angiography of the chest successfully demonstrated the patency and quantified the caliber of the internal mammary vessels to support free flap breast reconstruction. Conclusions: Free tissue transfer is a viable option for breast reconstruction in patients with Poland syndrome undergoing mastectomy guided by preoperative computed tomography angiography to characterize the internal mammary vasculature.

Poland syndrome is a rare but serious condition that can affect a child's development and quality of life. Understanding the clinical features of this syndrome and the risk factors that can cause it is essential for timely diagnosis and treatment. However, there is currently a limited number of scientific studies on this pathological condition, so further research in this area is relevant and requires attention. The aim of the study was to review the literature, clinical features of Poland syndrome in a child and to analyse the risk factors that can lead to the onset of this condition.Results of the study. Premature placental abruption is a condition in which the placenta, an organ that forms during pregnancy to provide nutrition and oxygen to the fetus, detaches from the uterine wall before the onset of labour. It disrupts the balance between the forces of the uterine and fetal muscles, which can lead to thrombosis, bleeding and other complications for both mother and fetus. This condition can be observed in any trimester of pregnancy and requires the attention of doctors for timely diagnosis and treatment. Poland syndrome is a congenital anomaly characterised by chest and muscle defects, as well as other developmental defects. According to the Foucras classification, there are three or four degrees of severity, depending on the level of asymmetry and deformation of the chest. Hypoplasia of the chest muscles, cardiovascular anomalies, hand and finger abnormalities are among the complications that can accompany CP. A clinical study revealed severe chest asymmetry and motor disorders in a child diagnosed with Poland syndrome (incomplete form). The risk factors for premature placental abruption in the mother were a history of previous pregnancy complications and a viral urogenital infection.Conclusions. Studying the clinical features of Poland syndrome and the risk factors leading to its occurrence is important for the timely diagnosis and treatment of this condition in children. Understanding these aspects can help improve the quality of life of patients and prevent complications.

Body Self-Perception After Breast Reconstruction in Young Female Patients Affected by Poland Syndrome

Background Cosmetic and social aspects of breast anomalies in Poland syndrome are not negligible. Early diagnosis and appropriate therapeutic timing may have a positive impact on quality of life. Methods Females affected by Poland syndrome, who had breast reconstruction between 2014 and 2018, were asked to complete the Body Uneasiness Test and the postoperative Breast-Q. Correlation between scores was evaluated. Correlation between scores was statistically evaluated. Results Thirty patients who had completed breast reconstruction at 20.9 ± 6.5 yo fulfilled the questionnaires at the average age of 26.5 ± 8.1 yo. BUT scores were similar to healthy population considering different age groups, with the exception of Compulsive Self-Monitoring subscale for 16–17-year age group. A correlation between Depersonalization and “Thighs” and “Legs” was present. On average, satisfaction with breast resulted 79.1%, satisfaction with surgical outcome was 94.9%, psychosocial well-being was 78.5%, sexual well-being was 75.3%, and relative physical well-being in chest and upper body was 36.9%. Global Uneasiness, Avoidance, Weight Phobia, Body Image Concern and Depersonalization were significantly correlated with lower psychosocial well-being. Avoidance was significantly correlated with lower sexual well-being. Conclusions Breast reconstruction in Poland syndrome can help to improve quality of life. However, general body uneasiness can affect satisfaction with the final result. Level of Evidence IV This journal requires that authors assign a level of evidence to each article.
