Japanese encephalitis (JE) is a serious health problem in the southwestern region of Nepal. Serological diagnostic kits for routine diagnostic use in this region have not been available. This study was performed to examine if the particle agglutination (PA) assay for Japanese encephalitis virus (JEV) IgM could be applicable to the samples collected in Nepal and also to evaluate the accuracy of clinical diagnosis of JE. One hundred and ninety-three blood samples were collected from the patients clinically diagnosed with JE or other infectious diseases in the JE-endemic, southwestern region of Nepal, in 2000. The PA assay was performed on these 193 serum samples and the results were compared with those by IgM-capture ELISA. Eighty-six samples were IgM-positive by the PA assay, and 71 of 86 were also positive by IgM-capture ELISA (sensitivity, 99%; specificity, 88%; positive predictive value, 0.82; negative predictive value, 0.99). These results suggest that the PA assay is a simple, reliable and useful diagnostic test to support clinical diagnosis in rural hospitals of Asia including Nepal.

Plasmid-encoded granulocyte-macrophage colony-stimulating factor (GM-CSF) is an adjuvant for genetic vaccines; however, how GM-CSF enhances immunogenicity remains to be elucidated. In the present study, it was demonstrated that injection of a plasmid encoding the premembrane (prM) and envelope (E) protein of Japanese encephalitis virus and mouse GM-CSF (pJME/GM-CSF) into mouse muscle recruited large and multifocal conglomerates of macrophages and granulocytes, predominantly neutrophils. During the peak of the infiltration, an appreciable number of immature dendritic cells (DCs) appeared, although no T and B-cells was detected. pJME/GM-CSF increased the number of splenic DCs and the expression of major histocompatibility complex class II (MHCII) on splenic DC, and enhanced the antigenic capture, processing and presentation functions of splenic DCs, and the cell-mediated immunity induced by the vaccine. These findings suggested that the immune-enhancing effect by pJME/GM-CSF was associated with infiltrate size and the appearance of integrin αx (CD11c)+cells. Chitosan-pJME/GM-CSF nanoparticles, prepared by coacervation via intramuscular injection, outperformed standard pJME/GM-CSF administrations in DC recruitment, antigen processing and presentation, and vaccine enhancement. This revealed that muscular injection of chitosan-pJME/GM-CSF nanoparticles may enhance the immunoadjuvant properties of GM-CSF.

INTRODUCTION: Japanese encephalitis (JE) is a disease of rural agricultural areas in the Terai regions of Nepal. The aim of the study was to compare particle agglutination assay (PA) with IgM capture ELISA.MATERIALS AND METHODS: The descriptive crosssectional study was conducted from August 2006 to September 2008. A total of 552 serum samples were collected from patients clinically diagnosed as acute encephalitis syndrome (AES) or viral fever in Tribhuvan University Teaching Hospital (TUTH) in Kathmandu, Lumbini Zonal Hospital (LZH) in Rupandehi and Bheri Zonal Hospital (BZH) in Banke districts. The samples were tested for Japanese encephalitis virus (JEV) specific IgM by two serological methods, IgM capture ELISA and PA.RESULTS: Among 552 samples, 258 samples were positive by PA and 205 were positive by IgM capture ELISA. The overall seroprevalence was 46.7% and 37.1% by the PA and IgM ELISA respectively. Two hundred fifty eight samples (47.7%) were positive for anti-JE IgM by PA assay and 200 samples (77.5%) of the 258 PA IgM positive samples were also IgM positive for anti-JE IgM by IgM-capture ELISA. Thus, PA assay had a sensitivity of 97.5% and specificity of 83.3% in comparison with IgM-capture ELISA. A positive predictive value of 0.77 and negative predictive value of 0.98 was observed with PA in comparison with IgM-capture ELISA.CONCULSIONS: The result from PA was highly compatible with IgM capture ELISA. Both sensitivity and specificity of PA was acceptable in comparison with IgM-capture ELISA
