Development of a case-mix funding system for adults with combined vision and hearing loss

Abstract: BackgroundAdults with vision and hearing loss, or dual sensory loss (DSL), present with a wide range of needs and abilities. This creates many challenges when attempting to set the most appropriate and equitable funding levels. Case-mix (CM) funding models represent one method for understanding client characteristics that correlate with resource intensity.MethodsA CM model was developed based on a derivation sample (n = 182) and tested with a replication sample (n = 135) of adults aged 18+ with known DSL who w… Show more

“…Since the interRAI assessments are used widely in Canada, and in multiple other countries, these data provide a cost-effective source of information for testing and validating QIs. Although their main function is to assess the overall care needs of the individual, in order to drive care planning, interRAI assessments are useful for case-mix measurement [ 66 69 ] and quality assessment [ 39 , 70 72 ]. In Canada, public reporting on select QIs already exists for long-term care homes [ 73 ], and those receiving PC and their families deserve a similar level of transparency.…”

“…These systems tend to have two main components: a) a classification system, based on clinical characteristics, to group service users into categories with similar levels of resource use; and b) a weighting system (case-mix indexes or CMIs) assigns ratiolevel numeric values to these groups that can, among other functions, be applied to funding formulas in payment systems. interRAI-based case-mix systems are available for nursing homes (307)(308)(309), home care (310), intellectual disability services (311,312), and intervener/interpreter services for dual sensory loss (313).…”
