Development and validation of the Early Pediatric Groningen Defecation and Fecal Continence questionnaire

Abstract: There are no compatible tools that assess bowel function in young children, older children, and adults. This precludes clinical follow-up and longitudinal scientific research. Our aim was therefore to develop and validate a bowel function questionnaire equivalent to the pediatric (8–17 years) and adult (≥ 18 years) Groningen Defecation and Fecal Continence (DeFeC) questionnaires for children from the age of 1 month to 7 years. We developed, validated, and translated the Early Pediatric Groningen DeFeC (EP-DeFe… Show more

Four common assessments for colorectal function, including (in)continence, in anorectal malformations and/or Hirschsprung's disease were the Rintala Bowel Function Scoring System 77 ( n = 9), Baylor Continence Scale (BCS) 78 ( n = 4), Cleveland Clinic Fecal Incontinence Severity Scoring System (or Wexner score) 81 82 ( n = 2), and the Krickenbeck criteria 102 ( n = 1).…”

“…We recommend annual assessment of voiding pattern using a detailed voiding history to identify symptoms of bladder dysfunction, such as the questionnaire developed by the Children's National Medical Center, offered by the National Kidney Foundation (https://www.kidney.org/sites/default/files/docs/voidhist.pdf). If the questionnaire suggests voiding dysfunction or if daytime continence is not achieved by age 5 years—the age of continence in typical children according to Verkuijl et al (2023), referral to a pediatric urologist is advised. In cases where clinical assessment is insufficient to determine the cause of voiding dysfunction, urodynamic studies should be considered.…”

“…We performed this cross-sectional study between April 2021 and August 2021. Parents/caregivers (hereafter referred to as parents) of children aged 1 month to 7 years were invited to complete the validated Early Pediatric Groningen Defecation and Fecal Continence (EP-DeFeC) questionnaire ( 13 ). The EP-DeFeC contains validated scoring systems and criteria regarding bowel and urinary functioning, but also questions regarding demographic characteristics, therapies, and the parents’ classification of their child’s bowel health ( 13 ).…”

“…Additionally, we examined the prevalence and severity of constipation, fecal incontinence, and urinary incontinence. To define these conditions, we used the validated scoring systems and criteria that are incorporated in the EP-DeFeC questionnaire ( 13 ).…”
