Development and Validation of a Prediction Model for Prehospital Triage of Trauma Patients

Abstract: IMPORTANCE Prehospital trauma triage protocols are used worldwide to get the right patient to the right hospital and thereby improve the chance of survival and avert lifelong disabilities. The American College of Surgeons Committee on Trauma set target levels for undertriage rates of less than 5%. None of the existing triage protocols has been able to achieve this target in isolation.OBJECTIVE To develop and validate a new prehospital trauma triage protocol to improve current triage rates. DESIGN, SETTING, AND… Show more

“…However, several studies from Norway have described considerable higher undertriage than the accepted 5% [5,9,10,16] and also considerable higher overtriage than accepted, of about 80% [10] using a one tier TTA model. A fresh Dutch study estimated considerable improvements in triage precision after identifying the most optimal triage criterias in their own trauma population [15]. Efforts have been made elsewhere to address patients > 65 years of age with low--energy falls, with expedited teams specifically addressed for this group.…”

“…To generate these characteristics we obtained access to baseline demographic and clinical data from a recent prehospital major trauma triage study by van Rein et al, using the data collected in the Central Netherlands region. [12] This provided a high quality data set, which should be representative of developed world patient. This data is summarised in Table 1.…”

“…Such a comment would not be made in today's smartphone era: use of a prediction calculator to assist in physician knowledge and the practice of medicine is increasingly common. 3 Miettinen 4(p441) -the professor of the aforementioned epidemiology class-wrote in his eminent series on the modern scientific physician that the practice of medicine is an art: "Like Plato originally, and just before him, Aristotle distinguished science from the arts-the latter meaning 'productive' arts (techne), medicine among these. The essential difference between science and the arts was in the nature of the product: knowledge (episteme) vs artifacts or actions."…”
