Development and Validation of a Risk Prediction Model for In-Hospital Mortality Among Patients With Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis—ABCD-10

Abstract: IMPORTANCE Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) is a spectrum of severe mucocutaneous drug reaction associated with significant morbidity and mortality. A previously developed SJS/TEN-specific severity-of-illness model (Score of Toxic Epidermal Necrolysis [SCORTEN]) has been reported to overestimate and underestimate SJS/TEN-related in-hospital mortality in various populations. OBJECTIVE To derive a risk prediction model for in-hospital mortality among patients with SJS/TEN and to comp… Show more

“…However, these scores were not validated in the context of immunotherapy-induced SJS/TEN. 5,6 For all cases listed in Table 1, the lowest estimated median SCORTEN and ABCD-10 SJS/TEN score were 2 and 3, representing a mortality rate of 12.1% and 25.5%, respectively. In our literature review, 23% of patients with SJS/TEN secondary to PD-1/PD-L1 inhibitors died after the onset of reaction.…”

“…He had a SCORTEN score of 3 and an Age, Bicarbonate, Cancer, Dialysis, 10% body surface area (ABCD-10) of 4. 5,6 These scores are used to estimate the mortality of patients presenting with SJS/TEN. Histopathologic examination of a skin biopsy showed a subepidermal blister with overlying prominent full thickness epidermal keratinocytic necrosis and a mild superficial perivascular infiltrate primarily of lymphocytes ( Figure 2).…”

“…In addition to SCORTEN, the ABCD-10 criteria (i.e., age, bicarbonate, cancer, dialysis, and 10% BSA) have been developed as an alternative risk prediction model for in-hospital mortality among patients with SJS and TEN ( Noe et al, 2019 ). However, similar to SCORTEN, this model has not been validated in pregnant patients.…”
