Development and Implementation of a Geriatric Care/Case Management Program in a Military Community-Based Family Medicine Residency

Abstract: This article discusses how the development of a longitudinal geriatric assessment form facilitated a case management program in identifying high-risk frail elders within a military family practice clinic. A careful review of geriatric assessment tools was performed. From this review, a model geriatric assessment form was developed. A "SWOT" (strengths, weaknesses, opportunities, and threats) analysis of the family medicine department was completed to determine if the environment was ready for case management. … Show more

“…SWOT analysis allows one to collect and analyse a great quantity of information about the strengths, weaknesses, opportunities and threats of a programme. It was originally developed for change management in business planning, but it is an easy tool that can adapted for use in many different situations [13][14][15]. SWOT analysis has been used in the medical field for programme development and strategic planning, for documenting service organization, to improve models of care and to assess the readiness for implementing new models of care [12,13,[15][16][17][18]].…”

An analysis can be completed solely by a programme manager but, when employed with multiple stakeholders, a SWOT analysis is well suited for participatory evaluation because it is based on actors' perceptions and can contribute to participants' awareness and empowerment, facilitating the development of commonly shared organizational goals [19].…”

“…[11] Strengths-Weaknesses-Opportunities-Threats (SWOT) analysis [12,13] is a QI tool used in studies throughout the world [12,[14][15][16][17][18] to inform improvement efforts in a variety of healthcare settings and populations such as traumatic brain injury, [14] pediatric rehabilitation, [15] community health, [16] oral healthcare, [17] and geriatric care in the military. [19] In our study, facility staff and Veterans' perspectives were col-lected to determine the strengths and weaknesses of the ECUs as well as their opportunities and threats. [14] SWOT analysis results provided insight on facility staff and Veterans' overall satisfaction with the ECU and resulting patient independence.…”

“…We used a questionnaire incorporating the four dimensions included in a SWOT analysis to identify (1) the strengths and (2) weaknesses of an organization, as well as (3) the opportunities and (4) threats that lie within its environment [26,27]. SWOT analysis has been deemed a useful tool to examine a rehabilitation program [28] and it has been widely used in various health-care settings [29], including voluntary community health movement [30], oral public health [31], and military medicine [32]. One individual (MEL), who is knowledgeable about TBI networks and is familiar with interview techniques, performed all interviews.…”
