Developing a Preference-Based Glaucoma Utility Index Using a Discrete Choice Experiment

Abstract: This study developed a preference-based utility measure (Glaucoma Utility Index) using the DCE approach. The index, estimated on the basis of 286 respondents, demonstrated both theoretical and convergent validity with other generic health outcome measures and measures of glaucoma severity. Further research investigating preferences by clinically defined glaucoma health status is indicated. Methodological research should focus on alternative methods of scaling for use within a generic Quality Adjusted Life Year… Show more

“…The one study that used the classification system of the non-preference-based measure without modification actually derived the non-preferencebased measure in the same study, and is therefore discussed in the section below on deriving measures 'de novo' . 45 Six papers provide a clear and detailed description of their chosen methodology, and all analysed the performance of items from the non-preference-based measure using existing data and used a selection of psychometric criteria to select a subset of items. 31,[50][51][52]58,60 Two papers also used qualitative analysis alongside the psychometric analysis.…”

“…Three papers used qualitative research, 47,49,57 two papers used a literature review, 46,55 three papers used a combination of literature review, patient interviews and expert opinion, 20,21,45 one paper used a combination of qualitative research and a variety of psychometric analyses 53 and one paper used psychometric analysis of a battery of existing items from the literature. 56 Qualitative research All three papers that used qualitative research to determine the classification system used a similar approach and conducted the process using recommendations by Babbie.…”

“…All but four of these studies aimed to produce a QALY measure, several of which stated that they selected their valuation technique in order to enhance comparability with the EQ-5D or to comply with guidelines [96] issued by the UK National Institute for Health and Care Excellence (NICE) [23; 30; 42; 46; 47; 57; 61; 82; 84; 87; 89]. Three studies selected DCE methods to produce non-QALY instruments to inform decision analysis, stating that DCEs mirror the ways in which patients choose between treatment strategies [25; 28] or are more appropriate than SG or TTO approaches for chronic conditions where death is not a likely outcome [14]. The latter also provided a rationale for using DCEs to elicit preferences for a QALY measure [24].…”

“…The EQ-5D data obtained from 255 OAG individuals from Leeds and Aberdeen as well as members of the International Glaucoma Association were used. 277 As the degree of visual impairment with mild OAG is minimal, it was assumed that the score for those with OHT would be the same as the score for those with mild OAG. This value is comparable to the mean age group population EQ-5D score 276 that would be estimated for the model-simulated cohort age group.…”
