Determining the Safety and Effectiveness of Electrocautery Enhanced Scissors for Peroral Endoscopic Myotomy (with Video)

Abstract: Background/Aims: Peroral endoscopic myotomy (POEM) has recently come to the forefront in the management of achalasia. We aimed to analyze the efficacy and safety of the use of electrocautery enhanced scissors (EES) for POEM. Methods: This retrospective cohort study prospectively collected the data of all adult patients (aged ≥18 years) with normal foregut anatomy who underwent POEM using EES. The patients' baseline characteristics and procedure details (time, tunnel length, myotomy length, depth, and location)… Show more

“…While the procedural technique has remained homogeneous, various electrosurgical knives continue to evolve to prevent injuries to the surrounding structures and reduce the need for device changes. 2 , 3 , 4 , 5 A novel device called the endoscopic dissector, used for endoscopic submucosal dissection, 6 , 7 can serve the function of blunt dissection as well as hemostasis (approved for use by the U.S. Food and Drug Administration); but there is no report of its use during POEM in the published literature. In our case, we demonstrate using an endoscopic dissector while performing POEM for achalasia.…”

“…Inoue et al 15 introduced the concept of natural orifice transluminal endoscopic surgery (NOTES) 16 17 associated with submucosal dissection 18 and named this technique peroral endoscopic myotomy (POEM). Studies have shown that the short- to mid-term efficacy of POEM is greater than 90 % and that the procedure is associated with less postoperative pain 19 20 21 22 23 24 25 . However, this procedure is frequently associated with some common adverse effects, such as pneumoperitoneum and surgical emphysema 26 .…”
