An evaluation of enzyme immunoassays for determination of thyroxine in serum (EMIT ABA thyroxine assay, Syva Corp., ENZYMUN assay thyroxine, Boehringer Mannheim) and of thyroxine binding index (ENZYMUN assay TBI 1 ), Boehringer Mannheim) is presented. The precision of the enzyme immunoassays was adequate (coefficients of variation ranged from day to day with EMIT from 3-11% and with ENZYMUN from 4-11%). Both assays are specific and easy to perform. About 20 unknown samples can be analyzed in duplicate by EMIT within 60 minutes and by ENZYMUN within 250 minutes. A comparison of the results obtained by enzyme immunoassays and radioimmunoassay in a series of about 100 patients showed a good correlation between both methods. The precision of the ENZYMUN TBI assay was adequate (coefficient of variation from day to day 4.9%) and the thyroxine/TBI-ratio correlated well with the thyroxine/TBG-ratio. Erprobung von EnzymZusammenfassung: Es wird über eine Erprobung von Enzym-Immuno-Tests zur Bestimmung von Thyroxin im Serum (EMIT ABA Thyroxin Test, Syva Corp., ENZYMUN-Test Thyroxin, Boehringer Mannheim) und des Thyroxin-Bindungs-Index (ENZYMUN-Test TBI 1 ), Boehringer Mannheim) berichtet. Die Präzision der Enzym-Immuno-Tests war ausreichend (die Variationskoeffizienten lagen von Tag zu Tag mit EMIT bei 3-11% und mit ENZYMUN bei 4-11 %). Beide Tests sind spezifisch und rasch durchführbar. Etwa 20 unbekannte Proben können als Doppelbestimmungen mit EMIT in 60 Minuten und mit ENZYMUN in 250 Minuten durchgeführt werden. Ein Vergleich der Ergebnisse, welche mit den Enzym Jmmuno-Tests und einem Radioimmunotest bei einer Serie von etwa 100 Patienten erhalten würden, zeigte eine gute Übereinstimmung beider Methoden. Die Präzision des ENZYMUN TBI-Tests war ausreichend (Variationskoeffizient von Tag zu Tag 4,9%) und der Thyroxin/TBI-Quotient korrelierte gut mit dem Thyroxin/TBG-Quotienten.either on the EMIT technique (7-8) (EMIT thyroxine FT i -f ree thyroxine index, T 4 = thyroxine, T 3 = triiodoassay, Syva Corp., Palo Alto, U.S.A.) or on an ELISA thyronine.

Summary: A gas Chromatographie procedure for the simultaneous determination of carbamazepine, phenobarbital and phenytoin using SP 2510 DA as stationary phase is presented. The antiepileptic drugs are determined simultaneously without derivatisation under isothermal conditions by a flame ionisation detector. The gas Chromatographie procedure can be easily mechanized. The coefficient of variation for the precision from day to day is 8.9% for carbamazepine, 7.0% for phenobarbital and 4.3% for phenytoin as calculated from single determinations. The deviations from the target value of spiked pool sera range from 4.3 to 9.5%. The gas Chromatographie results of this method and of determinations by Dexsil 300 for primidone and SP 1000 for ethosuximide are compared with the corresponding enzyme immunoassays (EMIT). The precision of the gas Chromatographie methods is somewhat better than EMIT. When patients' sera are analysed by both procedures the results show no clinically relevant differences. It can be concluded that the different methods are interchangeable and may be selected according to practical necessities. Kontrolle der therapeutischen Serumkonzentrationen von Antiepileptika mit Hilfe eines neuen gaschromatographischen Verfahrens und Enzymimmunotests (EMIT): Eine VergleichsstudieZusammenfassung: Ein gaschromatographisches Verfahren zur gleichzeitigen Bestimmung von Carbamazepin, Phenobarbital und Phenytoin an der stationären Phase SP 2510 DA wird beschrieben. Der Variationskoeffizient für die Präzision von Tag zu Tag beträgt bei Einzelbestimmungen 8,9% fur Carbamazepin, 7,0% für Phenobarbital und 4,3% für Phenytoin. Die Abweichungen vom Sollwert aufgestockter Proben schwanken zwischen 4,3 und 9,5%. Die Methode zeichnet sich durch eine besonders gute Praktikabilität aus, da gleichzeitig 3 häufig verordnete Antiepileptika isotherm, ohne Derivatbildung gemessen werden können. Die Ergebnisse dieses gaschromatographischen Verfahrens, sowie die Resultate der Primidonbestimmung an Dexsil 300 und der von Ethosuximid an SP 1000 werden mit den entsprechenden Enzymimmunoassays (EMIT) verglichen. Die Präzision der gaschromatographischen Methoden ist ähnlich oder besser als von EMIT. Bei der Analyse der gleichen Patientenseren mit den verschiedenen Analysenverfahren finden sich gewöhnlich keine klinisch relevanten Unterschiede. Die Bestimmungen können also je nach den praktischen Erfordernissen und apparativer Ausstattung wahlweise mit Hilfe der Gaschromatographie oder mittels EMIT durchgeführt werden.

In this article, many of the reports which describe the various assay procedures for 8 of the most commonly monitored drugs in plasma (digoxin, gentamicin, phenobarbitone, phenytoin, procainamide, quinidine, salicylates and theophylline) are reviewed, together with studies dealing with interferences of other drugs with these assays. Factors which are evaluated include whether the interference was studied when the drug was taken by a patient or a volunteer or by adding it to serum in vitro, the concentration or dose of the interfering drug (when reported), and the clinical implications of the interference. Suggestions as to how to eliminate some of these potential sources of interference are made.
