Objectives: Autoimmune hepatitis (AIH) is a necroinflammatory liver disease of unknown etiology. It has been proposed that certain herbs such as black cohosh and dai-saiko might trigger AIH. Khat is an ever green tree whose leaves have been chewed by people in Yemen where AIH is common. Aim of present study was to measure antinuclear antibodies (ANA) and anti-smooth muscle antibodies (ASMA) as serum markers for AIH type1 in healthy people who chew and do not chew khat. It also aimed to determine some other risk factors for ANA and ASMA positivity. Methods: A total of 100 healthy individuals were enrolled in this study. They were divided into: Daily khat chewers and non-khat chewers. Three ml peripheral blood was withdrawn from each participant. Blood samples were tested for ANA and ASMA using indirect immunofluorescence assay. Results: The age of khat chewers ranged from 30-60 years with mean age 40.4±7.6 years. Non-khat chewer's age ranged from 30-57 years with mean age 39.9±6.2 years. The majority of khat chewers and non-chewers were in age groups 30-39 and 40-49 years old. There is no association between khat chewing and ANA or ASMA serum positivity (χ 2 =0.33, P=0.39 and χ 2 =1.5, P=0.16; respectively). Khat chewing has no association with ANA and ASMA positivity. Nevertheless, ANA and ASMA positivity has an association with aging. Conclusion: Study concludes that Khat chewing has no association with ANA and ASMA positivity. Nevertheless, ANA and ASMA positivity has an association with aging.

Introduction. Diabetes is a growing social and epidemiological problem. Accordingly, the incidence of complications associated with diabetes can cause a persistent high percentage of diseases of the cardiovascular system, kidney and nervous systems, and impaired vision. Objective. The aim of the study was to evaluate the incidence of immunological markers in patients with type 1 diabetes and those with type 2 diabetes: the anti-GAD, ANA, AMA, ASMA, APCA and LKM, compared to healthy people. Another objective of the study was to evaluate the correlation between their presence and the degree of metabolic control in both groups. Materials and methods. The study comprised 100 subjects aged 25-75 years with a body mass index (BMI) between 20-30 kg / m2, hospitalized in the Department of Internal Diseases, Diabetology and Endocrinology, at the Medical University of Warsaw, with previously diagnosed diabetes who were assigned to one of 2 groups (50 subjects with type 1 diabetes and 50 subjects with type 2 diabetes). The control group consisted of 21 healthy individuals without the diagnosis of diabetes and a prediabetic state. All the study participants had the examined antibodies determined along with the panel of biochemical tests and neurological examination for diabetic neuropathy and fundus examination. Results. Anti-GAD antibodies were present in both groups of patients. Their presence was found in 30% of people with type 1 diabetes and in 16% of people with diabetes type 2. The presence of ANA antibodies was found in 24% of people with type 1 diabetes and 22% of people with type 2 diabetes. There was no correlation between the presence of ANA antibodies ANA and duration of diabetes. In the group of patients with type 1 diabetes, there was a correlation between the presence of ANA and the incidence of diabetic polyneuropathy. ASMA and APCA antibodies occurred with equal frequency in both studied groups (4% vs. 10%). There were no antibodies of AMA or anti-LKM in any of the patients. Conclusions. Marking of ANA antibodies in patients with type 1 diabetes may be a marker used to isolate a group of patients at risk of developing diabetic neuropathy. The presence of anti-GAD in type 2 diabetes may be a LADA marker which specifically marks the group of patients with type 2 diabetes, in whom there is a faster metabolic death of beta cells. The current classification of diabetes is vague, and in the near future it should be modified based on specific patient characteristics, phenotypic appearance, as well as the results of additional tests. Determination of antibodies AMA, ASMA, APCA and anti-LKM does not seem to be significant in the diagnosis of diabetes and its chronic complications.

Özet ABS tRACtAim: Urticaria is a common disease in children; however, there are few examples in the literature about childhood urticaria. In this study, we aimed to investigate the various forms of urticaria in children along with the clinical features and etiological factors consistent with this condition. Materials and Methods:We retrospectively analyzed medical records of 125 children with urticaria, aged ≤18 years, who attended outpatients clinics. We investigated the etiological factors contributing to urticaria, such as infections, drugs and autoimmune disorders. Results: Of the 125 patients, 27 had chronic urticaria (CU) and 98 had acute urticaria (AU). The mean age at disease onset was 10.4±5.4 years in CU and 5.74±4.6 years in AU, the difference was statistically significant (p<0.001).Infections were found to be the first cause (43.9%) of AU. Fifty-one percent (n=50) of AU were found to be idiopathic. Of the CU patients, 9 (33.3%) had inducible urticaria. Infections were found to be the first cause (14.8%) (n=4) of CU. Seventy-six point two percent of chronic spontaneous urticaria were found to be idiopathic. Helicobacter pylori (HP) antigen positivity was found in 43.8% (7/16) of the CU cases. Four to five of children showed remission after HP eradication. Conclusion: AU is the most common form of urticaria seen in childhood and infections appear to be the most important causative factor for AU. HP is an important infectious agent leading to CU. Amaç: Ürtiker çocukluk çağında sık görülse de literatürde çocukluk çağı ürtikeri ile ilgili çalışma az sayıdadır. Biz bu çalışmada çocukluk çağı ürtikerinin klinik tip ve özelliklerini ve altta yatabilecek etiyolojik nedenleri saptamayı amaçladık. Gereç ve Yöntemler: Polikliniklerimize başvurup, ürtiker tanısı almış 18 yaş ve altı 125 hastanın poliklinik kartları retrospektif olarak incelendi. Ürtiker etiyolojisinde yer alabilecek enfeksiyonlar, ilaçlar, otoimmün hastalıklar gibi etiyolojik faktörler araştırıldı. Bulgular: Çalışmaya alınan 125 hastanın 27'si kronik, 98'i akut ürtikerli idi. Kronik ürtikerde (KÜ) hastalık başlangıç yaş ortalaması 10,4±5,4 yıl, akut ürtikerde ise 5,74±4,6 yıl idi ve aradaki fark istatistiksel olarak anlamlı bulundu (p<0,001). Akut ürtikerde en sık etiyolojik faktör olarak infeksiyonlar (%43,9) saptandı. Akut ürtikerin %51'i (n=50) idyopatik idi. KÜ olguların 9'unda (%33,3) indüklenebilir ürtiker mevcuttu. KÜ etiyolojide ilk sırada enfeksiyonlar (%14,8) (n=4) yer almaktaydı. Kronik sponton ürtikerli olguların %76,2'si idyopatik idi. KÜ dışkıda Helicobacter pylori (HP) antijeni %43,8 (7/16) pozitif saptandı ve tedavi edilen 4/5 olguda remisyon elde edildi. Sonuç: Çocuklarda en sık akut ürtiker formu görülmektedir ve etiyolojide enfeksiyonlar önemli yer tutmaktadır. KÜ HP enfeksiyonu etiyolojide yer alabilen önemli bir enfeksiyöz etkendir.
