Determinants of quality of life at first presentation with schizophrenia

Abstract: Reducing the duration of untreated psychosis may have a beneficial effect on the subsequent QOL of patients presenting with schizophrenia. First-episode patients with a protracted duration of untreated psychosis or impaired premorbid adjustment may warrant specific treatment interventions to prevent the development of secondary handicaps.

“…All participants reported at least moderate levels of negative symptoms at baseline, so it is unclear whether these findings would be replicated in participants with lower negative symptoms. In addition, most participants reported a long duration of illness, which may be an issue given the impact of symptoms on SQOL is more prominent in the early stages of illness (Browne et al, 2000). In this study no significant difference was detected between participants with a long and short illness duration.…”

“…46 A longer duration of untreated psychosis is correlated with a worse QoL, [47][48][49] worse treatment outcome 50 and worse prognosis. 6 QoL tends to be lower where people with psychosis are single, 51 have psychiatric co-morbidity, 51 poor premorbid adjustment, 49 longer duration of psychotic symptoms 49 and poor social relations and finances. 52 From a service user's perspective, being an NHS inpatient has been described as "horrible, scary, surviving the system, institutionalised, feeling strange, labelled, used in experiments, no choice".…”

“…Research to date has focused on examining relationships between QoL and key symptom-related factors. For example, QoL in early psychosis has been found to be related most significantly to depression [35], anxiety [32], general psychopathology and negative symptoms of psychosis [6]. However, there is less evidence of a relationship between QoL and positive psychotic symptoms [16].…”
