A Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) causing coronavirus disease 2019 (COVID-19) led to a pandemic outbreak in 2019. COVID-19’s course and its treatment in immunocompromised patients are uncertain. Furthermore, there is a possibility of protracted SARS-CoV-2 infection and the need for repeated antiviral treatment. Monoclonal antibodies against CD20, which are used, among other things, in the therapy of chronic lymphocytic leukaemia and follicular lymphoma, can induct immunosuppression. We present a case report of a patient with follicular lymphoma, treated with obinutuzumab, who was diagnosed with prolonged, ongoing SARS-CoV-2 infection and related organizing pneumonia. The recognition and the treatment were challenging which makes this case noteworthy. Antiviral therapy with several medications was administrated to our patient and their temporary, positive effect was observed. Moreover, high-dose intravenous immunoglobulin was applied, because slowly decreasing IgM and IgG levels were observed. The patient also received standard treatment of organizing pneumonia. We believe that such a complex approach can create an opportunity for recovery. Physicians should be conscious of the course and treatment possibilities facing similar cases.

SummaryThis paper presents a comprehensive three‐step approach (CNN‐JSO‐LSTM) for predictive modeling using a pandemic such as COVID‐19 as a test case. Initially, a Convolutional Neural Network (CNN) is employed to extract crucial features pertinent to the pandemic. Subsequently, the Jellyfish Search Optimizer (JSO) algorithm is applied for feature selection, identifying the most relevant factors. These chosen features are then inputted into a Long Short‐Term Memory (LSTM) network, responsible for classifying samples into “healthy” and “diseased” categories. Our method enhances LSTM performance using the Jellyfish Search optimizer, resulting in exceptional prediction accuracy. Our experiments achieved remarkable metrics, with an accuracy of 95.32%, high sensitivity (94.87%), and precision (94.28%), surpassing alternative methods. In conclusion, our study presents a promising and highly accurate approach for pandemic prediction, harnessing deep learning and swarm intelligence techniques. These findings suggest a potential for more effective pandemic management and intervention strategies.

The impact of community-acquired respiratory viral infections (CARVIs) in the transplant population has been highlighted by the recent coronavirus disease 2019 (COVID-19) pandemic. Patients undergoing haematopoietic stem cell transplantation (HCT) following acute COVID-19 experienced significant morbidity and mortality. Few guidelines and reports suggest that HCT is deferred during persistent severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) positivity during and after infection, and is feasible only in fully recovered patients [1,2]. However, those with high-risk haematological malignancies often progress and succumb to their disease without timely HCT. These two opposing pressures must therefore be reconciled, yet data are sparse on the optimal timing and safety of HCT that needs be performed during or immediately following acute COVID-19 infection.
