Detection of tumor cells in bone marrow of patients with primary breast cancer: a prognostic factor for distant metastasis.

Abstract: This study suggests that tumor-cell detection in bone marrow of patients with primary breast carcinoma is a good predictor for all distant relapses (P < .0005, Cox multiple regression analysis) and provides additional information in regard to other prognostic factors. The highest predicting value for distant metastasis results from the combination of nodal status, negative PR, and tumor-cell presence in bone marrow.

“…Therefore, only small cases series of patients with solid cancers and bone marrow metastases are reported in the literature. Among these reports, malignancies of the prostate, breast, lungs, and stomach were the primary tumors that most frequently metastasized to the bone marrow (Hansen et al, 1971;Shah et al, 1985;Ceci et al, 1988;Tritz et al, 1989;Diel …”

“…Various techniques, including morphology, flow cytometry and cytogenetics, have been used to detect disseminated tumour cells, but most of these have limited sensitivity and specificity (Frank et al, 1990;Diel et al, 1992;Johnson et al, 1995) and are time-consuming (Gross et al, 1995). Immunocytochemistry has also been used in an attempt to detect circulating breast carcinoma cells perioperatively, but sensitivity remains a concern with this technique (McCulloch et al, 1995;Choy and McCulloch, 1996).…”
