Detection of soft‐tissue abscess: Comparison of diffusion‐weighted imaging to contrast‐enhanced MRI

Abstract: Purpose: To compare the diagnostic performances of diffusion-weighted imaging (DWI)-combined magnetic resonance imaging (MRI) performed without intravenous contrast material with gadolinium contrast material-enhanced (CE) MRI for diagnosing soft-tissue abscesses. Materials and Methods: In all, 119 patients (mean age: 56 years) with skin and soft-tissue infection who underwent contrast-enhanced MRI with DWI (b 5 0-800) were included. Two readers independently reviewed both image setsnonenhanced conventional MR … Show more

“…Abscesses and thrombi impede the diffusivity of water molecules because they have a hyperviscous nature [30,31]. The median ADC value (0.877 × 10 −3 mm 2 /s) of abscesses was significantly lower than that (2.118 × 10 −3 mm 2 /s) of phlegmon (p < 0.001) and that (3.008 × 10 −3 mm 2 /s) of edema (p < 0.01) [32]. In our article, the ADCs of lung cancers were distributed over 0.9 to 1.6 × 10 −3 mm 2 /s.…”

“…Some pathologic processes such as pulmonary tuberculosis, nontuberculous mycobacteria, pulmonary abscesses, sarcoidosis, chronic pneumonia, scars, and other inflammatory or infectious conditions behave like malignant lesions by exhibiting diffusion restriction [ 19 , 20 , 21 ]. ADC values of abscesses are low, and it was mentioned that median ADC value (0.877 × 10 −3 mm 2 /s) of abscesses was significantly lower than that (2.118 × 10 −3 mm 2 /s) of phlegmon ( p < 0.001), and that (3.008 × 10 −3 mm 2 /s) of edema ( p < 0.01) [ 22 ]. There were several reasons for the restricted diffusion of PAMI.…”

“…However, contrast-enhanced images are extremely helpful for specific diagnostic tasks, such as delineating a necrotic, non-enhancing bone, or rim enhancing fluid collections, such as abscess and sinus tract. Diffusion-weighted imaging helps in characterizing fluid collections and adds certainty in the diagnosis of abscess, which can be particularly useful in patients with contraindications to intravenous gadolinium based contrast [93,94]. Diagnosis of the osteomyelitis in a setting of neuropathic arthropathy is particularly challenging.…”
