In this cadaveric study of anterior plating of the distal radius, we aimed to determine the interobserver agreement and diagnostic performance for detecting dorsally protruding screws using the dorsal tangential radiographic view before and after specific training. Without prior instruction, 13 observers interpreted the dorsal tangential view of cadaveric specimens, in which anterior radial plates were placed. After seeing a training video on the dorsal tangential view, they repeated the task. Though we found that accuracy and interobserver agreement was lower than described in some other clinical series, training led to statistically significant improvements of (1) the interobserver agreement on the decision to exchange screws, (2) the self-confidence of the surgeon in obtaining adequate views, and (3) the number of fluoroscopic images required to obtain these views. After training, the number of protruding screws missed was reduced by 36%, but 7% of dorsally protruding screws was still missed.

Bone fracture has become more common in this era of vehicular modernization. Internal fixation has been used in the treatment of such cases. The use of screw and plate in internal fixation has been proven to provide more stability to fracture to promote better healing. The use of screw also carries certain risk such as self-loosening that can cause non-union and prolong healing. The complication of selfloosening is studied to comprehend the danger of self-loosening in screw-bone implant. The review on self-loosening in bolted joint is done in this paper to gain better understanding on the nature of self-loosening phenomena. The factor that affects the screw selfloosening in bone is further studied in the screw-bone implant environment. The identification of the screw loosening can be done with radiography, CT scan, MRI and ultrasound. This article have done a brief review on the complication of screw self-loosening, the cause of screw loosening in bolted joint and screw-bone implant and the assessment of screw loosening done by ultrasound and radiography.

In order to fix a fracture in osteosynthesis, it is necessary to attach screws bicortically to the bone. The length of the screws must be selected correctly in 1-mm increments: otherwise, injury to the surrounding tissue structure or insufficient fixation will result. The drill channel length can only be determined preoperatively to a limited extent and with insufficient accuracy and is therefore determined intraoperatively with a mechanical caliper gauge. This length determination is error-prone, which often leads to a false screw selection and at the same time to considerable complications in the healing process. A novel approach based on a sensory drive train was pursued, with which all mechanical drilling parameters were recorded and evaluated in combination with a length measurement that allows for determining the drill channel length. In order to overcome the limitations of previous drill concepts, a precise length measurement of the drill channel was introduced. The amplitude of a stimulated linear oscillation of the drill was monitored for drilling channel length measurements in order to reliably detect the beginning of the drilling process. The method provides the information required for handheld drilling without the limitation of constant drilling parameters. With initial results from laboratory tests with pig bones, the measurement method for the drill channel length has been validated on a test bench of the drilling machine. With the laboratory tests, a measurement uncertainty of 0.3 mm was achieved, so screws with a 1-mm step width can be reliably selected.
