Antimicrobial resistance among bacteria has constantly increased over the years with emergence of novel resistance mechanism among these versatile organisms. Identifying the resistance pattern of the isolates both regionally and globally is essential for the institution of appropriate antimicrobial therapy. This study was conducted to assess the prevalence of beta-Lactamase enzymes among species of Enterobacteriaceae.312 consecutive isolates belonging to the family of Enterobacteriaceae were included in the study conducted over a period of one year. After preliminary antibiotic susceptibility testing using standard guidelines, production of various beta-Lactamase enzymes was assessed by phenotypic methods.22.76% of the isolates were found to be Extended-Spectrum Beta-Lactamase producers, 37.82% were AmpC Beta-Lactamase producers and 7.37% of the isolates produced the Carbapenemase enzyme.Our study estimates the prevalence of various beta-Lactamase enzymes in isolates of Enterobacteriaceae in a tertiary care centre. The increase in production of these enzymes among bacteria necessitates the implementation of strict adherence to antibiotic policy and infection control measures.

Amaç: Karbapenem direnci, yüksek mortalitesi ve sınırlı tedavi seçenekleri nedeniyle önemli bir halk sağlığı sorunu haline gelmiştir. Bu nedenle karbapenem direnç mekanizmasını kısa sürede saptayan güvenilir ve basit testlere ihtiyaç duyulmaktadır. Bu çalışmanın amacı, karbapenem dirençli Klebsiella spp. ve E. coli izolatlarında karbapenemaz, GSBL ve AmpC varlığını aynı plak üzerinde disk difüzyon, kombine disk ve disk antagonizma test yöntemleri ile belirlemektir. Yöntem: Çalışmaya Aralık 2017 ile Aralık 2018 tarihleri arasında bir üniversite hastanesinde rutin hasta kültür örneklerinden izole edilen karbapenem dirençli Klebsiella spp. ve Escherichia coli izolatları dahil edildi. European Committee on Antimicrobial Susceptibility Testing önerileri doğrultusunda GSBL, KPC-tip ve metalobeta-laktamaz için doğrulama testi olarak kombine disk yöntemi ve OXA-48 tipi karbapenemaz için temosilin disk difüzyon testi uygulandı. Plazmit aracılı AmpC beta-laktamaz tanımlaması için disk difüzyon testi ve indüklenebilir (kromozomal) AmpC beta-laktamaz için disk antagonizma testi kullanıldı. Kontrol suşu olarak KPC-2 tipi karbapenemaz üreten BAA-1705 K. pneumoniae kullanıldı. Bulgular: Karbapenem direnci 56'sı (%74,6) Klebsiella pneumoniae, 17'si (%22.6) E.coli, biri (%1.3) Klebsiella ozaenae ve biri (%1.3) Klebsiella aerogenes olmak üzere toplam 75 izolatta tespit edildi. Metallo-beta-laktamaz tipi karbapenemaz ile ilişkili direnç 24 (%31.5) izolatta, GSBL 17 (%22.3) izolatta, AmpC 4 (%5.2) izolatta ve OXA-48 ile ilişkili direnç 31 (%40.7) izolatta belirlendi. Sonuç: Direnç mekanizmalarının hızlı tespiti uygun antimikrobiyal tedavi ve enfeksiyon kontrol önlemleri açısından önemlidir. Çalışmada uyguladığımız yöntem, kolay ve uygulanabilir bir algoritma oluşturmaktadır.

Способность Klebsiella pneumoniae, выделенной при внутрибольничных инфекциях, к синтезу -лактамаз С. А. АТАКИШИЗАДЕ Азербайджанский медицинский университет, кафедра инфекционных болезней, Баку, Азербайджан В последние годы отмечается увеличение штаммов бактерий, способных синтезировать -лактамазные ферменты широкого спектра (extended-spectrum -lactamase (ESBL). В работе изучена способность штаммов K. pneumoniae, выделенных от 30 больных в хирургических отделениях при инфекциях мочевыводящих путей (18), из областей хирургического вмешательства (7) и внутрибольничных пневмониях (5). Синтез ESBL K. pneumoniae был выявлен посредством фенотипического теста и с использованием двух дисков. ESBL-синтезирующая способность K. pneumoniae обнаружена у 40% выделенных внутрибольничных штаммов. Установлено, что ESBL-позитивные штаммы K. pneumoniae достоверно чаще (р < 0,05) изолировались при инфекциях в области хирургического вмешательства и пневмонии (86% и 60% соответственно). Ключевые слова: внутрибольничные инфекции, K. pneumoniae, -лактамазные ферменты, extended-spectrum -lactamase (ESBL)In recent years, there has been an increase in bacterial strains capable of synthesizing extended-spectrum -lactamase (ESBL). The study examined the ability of K. pneumoniae strains isolated from 30 patients in the surgical ward with urinary tract infections (18), from the areas of surgical intervention (7) and nosocomial pneumonia (5). Synthesis of ESBL K. pneumoniae was detected by means of a phenotypic test and using two disks. The ESBL-positive K. pneumoniae was found in 40% of the isolated nosocomial strains. It was established that ESBL-positive strains of K. pneumoniae were significantly more often (p <0.05) isolated from infections in the area of surgical intervention and pneumonia (86% and 60%, respectively).
