Detection of HCV by PCR in serum and PBMC of patients with hepatitis C after treatment

Abstract: Although hepatitis C is mainly hepatotropic, some studies suggest that hepatitis C virus (HCV) infects peripheral blood mononuclear cells (PBMC), using them as a reservoir, which might contribute to the development of resistance to treatment. Fifty-four hepatitis-C patients, who had been submitted to treatment, were selected. Blood samples were collected on the same day for the detection of HCV RNA in serum and PBMC by PCR, using the Amplicor HCV 2.0 assay (Roche Diagnostics). HCV genotyping was performed usin… Show more

“…The 100% sensitivity and 96.7% specificity found for the PBMCs PCR validated this test as a potential diagnostic tool for HCV infection. These results are in full agreement with previous reports 15. The 3.3% of negative subjects who tested positive by PBMCs PCR might be considered cryptogenic OCI as previously reported by other investigators 1719.…”

“…All patients with positive HCV SRT-PCR are also positive on PBMCs PCR. However, some patients with positive PBMCs PCR may have negative SRT-PCR 15. Follow-up of treatment-experienced HCV patients who presented with negative SRT-PCR and positive intra-PBMCs strand infection revealed HCV RNA seroconversion within 1 year 2.…”

“…In fact, extrahepatic reservoirs have important implications for transmission, disease progression, and effective treatment of HCV-infected patients. Clearance of HCV following antiviral treatment does not mean eradication of the virus from whole reservoirs (22). The presence of HCV RNA in PBMCs may lead to HCV reactivation in patients with SVR under special circumstances, such as immunosuppression (23).…”

“…The first report about HCV was reported in 1992 by screening 45 patients from the northern part of Pakistan [9]. There are six major genotypes of HCV and more than one hundred subtypes reported throughout the world [10]. Out of these, the genotype-1, genotype-2 and genotype-3 are most popular genotypes in terms of prevalence [10].…”
