Detection of extended-spectrum β-lactamase (ESBL)-producing Enterobacteriaceae using the MALDI Biotyper Selective Testing of Antibiotic Resistance–β-Lactamase (MBT STAR-BL) assay

Abstract: 32The MALDI Biotyper Selective Testing of Antibiotic Resistance-β-Lactamase (MBT 33 STAR-BL) assay, which analyzes bacterial induced hydrolysis of cefotaxime using 34 MALDI-TOF MS, correctly identified 100.0% of extended-spectrum β-lactamase 35 (ESBL)-producing Enterobacteriaceae as positive and 94.7% of non-ESBL producers 36 as negative in 80 strains tested.37 38

“…The Cica Beta Test 1/HMRZ-86/Chromogenic cephalosporin is a rapid kit test (generates results within 15 min) that is used for detecting ESBL in Gram-negative rods from primary culture [37]. Matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF) is another diagnostic tool that has been successfully used to detect ESBL [38]. Recently, the NG-Test CTX-M MULTI, a rapid immunochromatography technique (lateral flow), has proven to be useful for the detection of CTX-M-type enzymes (groups 1, 2, 8, 9, and 25), followed by the rapid identification of Enterobacterales in blood or urine samples using MALDI-TOF MS and flow cytometry [39,40].…”

“…[ 1 ] Twenty-three percent of E. coli infections and about 11% of infections by Klebsiella pneumoniae were caused by ESBL-producing bacteria. [ 2 ] The ESBLs can trigger an increase in antibiotic resistance, not only for β-lactam antibiotics but also against broad-spectrum cephalosporin and aztreonam. However, they can be blocked by β-lactamase inhibitors containing serine, such as clavulanate, avibactam, and sulbactam.…”

“…Previous studies have reported that MALDI-TOF MS can be used for identification of CRKP (Xu et al, 2017;Centonze et al, 2018;Zhang et al, 2019;Huang et al, 2020), MRSA (Kim et al, 2019;Wang H.-Y. et al, 2021), VRE (Griffin et al, 2012;Sabenca et al, 2020), and other strains (Sparbier et al, 2012;Kawamoto et al, 2019), but there are only few studies and the conclusions are inconsistent (Lasch et al, 2014;Brackmann et al, 2020;Paskova et al, 2020;Gato et al, 2021).…”
