Detection of Chlamydia trachomatis and Neisseria gonorrhoeae in Rectal and Oropharyngeal Swabs and Urine Specimens from Men Who Have Sex With Men with Abbott's M2000 RealTime

Abstract: We evaluated Abbott's RealTime assay for the detection of Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (GC) in the urethra, oropharynx, and rectum of 260 men who have sex with men. Compared with Hologic's AC2, RealTime had good agreement for detecting CT and GC. Overall, there were 25 CT and 44 GC AC2 positives, and 26 CT and 38 GC RealTime positives. For total negatives, there were 742 CT and 725 GC for AC2, 744 CT and 724 GC for RealTime.

“…Despite these limitations, the assay used to detect chlamydial DNA is widely used to detect genital infections. Previous studies have shown RealTime to be highly sensitive (95.3%) and specific (99.9%) for the diagnosis of C. trachomatis from urine and ocular swabs [ 13 , 28 , 29 ]. The RealTime assay has compared favorably to other nucleic acid amplification tests for detecting C. trachomatis DNA and RNA commonly used in trachoma research [ 13 , 28 , 30 ].…”

“…7779 Testing of rectal and oropharyngeal specimens should be routine in MSM, considered in women who are sexual contacts of gonorrhoea patients [1C], and be guided based on sexual history, risk and symptoms or signs in all other patients. 3,39,8089 Testing of pooled specimens (oropharyngeal, rectal, and urine/urogenital) could provide cost savings, but is not recommended, due to potentially decreased sensitivity, 3,39,9093 increased complexity, including risk of cross-contamination of sample, and lack of approval by FDA or other regulatory agencies. However, the evidence is limited and additional appropriate studies using different NAATs and evaluating cost and benefits (e.g.…”

“…For positive individual samples, the m2000 generated delta cycle threshold result was converted to Ct elementary body (EB) equivalent concentration based on a previously described calibration curve of known EB concentrations on the RealTime Assay. 27,28 Once Ct load was known for the positive individual samples, a total of 240 with the highest load were chosen for this project. Samples with sufficient Ct load, likely to obtain high quality full genome sequence data based on our previous studies, were re-extracted as described below.…”
