Detection of Anorectal Cancer among Patients with Crohn's Disease Undergoing Surveillance with Various Biopsy Methods

Abstract: Background/Aims: Crohn's disease (CD) is an inflammatory bowel disease. The risk of colorectal cancer (CRC) is increased in patients with CD. In Japan, anorectal cancer accounted for >60% of CRCs associated with CD. These anorectal cancers are typically diagnosed in advanced stages, because a surveillance protocol remains to be established. This study aimed to assess various biopsy methods for detecting CRC. Methods: This study included 72 patients (113 examinations) with CD who underwent cancer surveillance b… Show more

“…reported 24/64 (37%) right‐sided colon cancer cases and 40/64 (63%) left‐sided CRC cases. From the total review of 17 studies with 107 cases from Asia, we identified 90/107 (84.1%) patients with left‐sided CRC and 17/107 (15.9%) patients with right‐sided colonic cancer 15,25–40 . The locations of cancer in patients with CD were significantly different among countries ( P < 0.01).…”

“…The characteristics of 52 studies reporting data from 208 patients with ARC are shown in Table 2 11,15,19,23,25,28,30,34,36–80 . Although many reports did not describe the details of each characteristic, the median survival prognosis was 2.1 ± 2.3 years in 74 patients, and advanced cancer greater than stage T3 occurred in 46/74 (62.1%) patients.…”

“…A recent meta-analysis also showed a significantly worse prognosis with CD-CRC, but most studies have not included populations in Asia (6,24,25). In Western countries, CD-CRC tends to be more left-sided, similar to the findings in Asia, but the incidence of right-sided tumor is higher than the incidence of sporadic CRC (6,8,(26)(27)(28)(29). In this study, 81.5% of CD-CRC cases were located in the anorectum, similar to other reports from Asia (21,22).…”

“…In all of these cases, the disease duration was > 10 years, and abdominoperineal resection (APR) and total pelvic exenteration (TPE) were performed in most cases. Cancer surveillance revealed that the total detection rate of CRC was 6.19%, and the detection rate was 1.85% with core needle biopsy, 5.56% with endoscopic biopsy, and 5.88% with excisional biopsy [ 19 ].…”
