Detection by ultrasound of abnormality in twin pregnancies during the second trimester

Abstract: Five cases of abnormality in twin pregnancies, detected by ultrasound examination during the second trimester, are reported. In four pregnancies there was a fetal malformation: discordant anencephaly, a monozygotic, heterokaryotypic twin pregnancy with coexistant Turner's syndrome and intrauterine death, concordant body stalk syndrome, and a dicephalus monster. In the other pregnancy one fetus dies in utero to become a fetus papyraceous, Four of the pregnancies were associated with raised levels of maternal se… Show more

“…One of our cases has already been mentioned in a study reporting a series of monoamniotic twin pregnancies diagnosed at 11-14 weeks' gestation 7 . In four previous cases reported in the literature, the diagnosis was made in the second trimester and both twins were concordant for the anomaly [2][3][4][5] . The karyotype of the surviving twins was normal, as in all previous reports of body stalk anomaly.…”

“…In particular, the presence of this anomaly in monozygotic twins is an extremely unusual condition. To our knowledge, four cases of body stalk anomaly in a twin gestation have been reported so far [2][3][4][5] and in only two of them was the diagnosis made by ultrasound; the others were identified by maternal serum alpha-fetoprotein screening in the second trimester.…”

“…[8] Ultrasound scan is invaluable in the diagnosis of twin pregnancy and its complication. [9] The failure of serial ultrasound scan to detect fetuses papyraceous in the index case has also been reported elsewhere. [1011] This is thought to be due to anatomical position and technical difficulties.…”

“…Only three cases have been previously reported in the literature (Neilson et al, 1982;Mann et al, 1984;Heitt et al, 1992). The purpose of this paper is to report a case of concordant body stalk anomaly in monozygotic twinning and to discuss the 'early embryo cleavage disorder' in the pathogenesis of this condition.…”
