Designing the Cash and Counseling Demonstration and Evaluation

Abstract: The successful implementation of the CCDE (results from the evaluation are reported elsewhere) led to replication efforts in other states. The CCDE also inspired changes in Medicaid law and policy, including the 2002 "Independence Plus" Initiative by the Centers for Medicare and Medicaid and sections of the Deficit Reduction Act of 2005 intended to promote consumer-direction in Medicaid.

“…The Cash & Counseling Demonstration and Evaluation, the largest self-direction trial to date, was a randomized experiment involving over 6,700 Medicaid personal care beneficiaries with long-term service and support needs in three states (Doty, Mahoney, & Simon-Rusinowitz, 2007). In the Demonstration, those receiving self-direction consistently reported better quality of life, higher levels of life satisfaction, and reduced unmet disability-related needs (Carlson, Foster, Dale, & Brown, 2007).…”

“…Like the Cash and Counseling Program model [13], VD-HCBS provides Veterans with a monthly allotment based on an assessment of the extent of their need for assistance with personal care because of injury or the effect of disease. Veterans have the opportunity to spend their allotment on services that best meet their needs, such as personal care workers whom they hire and manage themselves, thus maximizing the concept of patientcentered care.…”

“…The CCDE compared cost, quality, and satisfaction of consumers receiving traditional personal care services with those receiving the cash option. (For further information about the CCDE study design, see Doty, Mahoney, & Simon-Rusinowitz, 2007.) The overall positive CCDE results led to a 12-state replication project.…”
