Application-centered image management provides access to image data including all relevant RT-specific elements. This approach, however, is not migration-safe, requires significant administrative work to ensure a redundancy level that protects against data loss and does not provide datasets that are linked to respective therapeutic interventions. Therefore, centralized image management and archiving that links images to patients and individual steps in the treatment pathway within a standardized DICOM(-RT) environment is preferable despite occasional problems with visualization of specific data elements.

[23][24][25] Early in the development of the RT-PACS, researchers recognized the existence of functional differences between the RT-PACS and the conventional radiology PACS. 26 For example, the high resolution required for the identification of pathologic abnormalities using the radiology PACS was not required for the RT-PACS, as the pathologies had already been identified in the radiology images.…”

Over the past 100 years, both diagnostic radiology and radiation therapy have grown from infancy to maturity. Accompanying this growth, the discipline of medical physics has evolved and advanced accordingly. New diagnostic and therapeutic procedures continue to be developed, for example, multidetector computed tomography, multileaf collimation, magnetic resonance imaging, dual-source computed tomography, and intensity-modulated radiation therapy. These are now incorporated in health care facilities throughout the world. Modern technologies such as these provide information on underlying pathology at increasingly higher resolutions, generating more information; thus requiring complex methods of image recording and storage. The management of the storage and retrieval of accumulated information is a domain of informatics. In this short review, we describe the different roles of medical physicists and the effective contribution of the American Association of Physicists in Medicine in the evolution of informatics. Medical physicists have contributed to the development of informatics in numerous ways, such as designing hospital information systems and infrastructures that better serve radiologists and other physicians. In addition, the positive exploitation of knowledge gathered in medical settings and effective interdisciplinary collaborations between scientists of different backgrounds have increased. These developments provide future medical physicists the opportunity to develop strategic roles in information technology and thus better contribute to health care.

The Use of Medical Images in Planning and Delivery of Radiation Therapy

The authors provide a survey of how images are used in radiation therapy to improve the precision of radiation therapy plans, and delivery of radiation treatment. In contrast to diagnostic radiology, where the focus is on interpretation of the images to decide if disease is present, radiation therapy quantifies the extent of the region to be treated, and relates it to the proposed treatment using a quantitative modeling system called a radiation treatment planning (RTP) system. This necessitates several requirements of image display and manipulation in radiation therapy that are not usually important in diagnosis. The images must have uniform spatial fidelity: i.e., the pixel size must be known and consistent throughout individual images, and between spatially related sets. The exact spatial relation of images in a set must be known. Radiation oncologists draw on images to define target volumes; dosimetrists use RTP systems to superimpose quantitative models of radiation beams and radiation dose distributions on the images and on the sets of organ and target contours derived from them. While this mainly uses transverse cross-sectional images, projected images are also important, both those produced by the radiation treatment simulator and the treatment machines, and so-called "digital reconstructed radiographs," computed from spatially related sets of cross-sectional images. These requirements are not typically met by software produced for radiologists but are addressed by RTP systems. This review briefly summarizes ongoing work on software development in this area at the University of Washington Department of Radiation Oncology.
