Desensitization in delayed drug hypersensitivity reactions – an EAACI position paper of the Drug Allergy Interest Group

Abstract: Drug hypersensitivity may deprive patients of drug therapy, and occasionally no effective alternative treatment is available. Successful desensitization has been well documented in delayed drug hypersensitivity reactions. In certain situations, such as sulfonamide hypersensitivity in HIV-positive patients or hypersensitivity to antibiotics in patients with cystic fibrosis, published success rates reach 80%, and this procedure appears helpful for the patient management. A state of clinical tolerance may be achi… Show more

“…A position paper on desensitization in non-immediate hypersensitivity has been published with protocols for antibiotics including beta-lactams attached [223]. • If there is a history of penicillin allergy, patients requiring a cephalosporin should undergo skin testing to both penicillin and to the specific cephalosporin.…”

“…Most published literature on drug desensitization pertains to immediate hypersensitivity reactions [1,2]. In the absence of controlled studies or an evidence-based review of desensitization in delayed hypersensitivity reactions (defined as onset more than 1 h after drug administration), experts have summarized their experience with regard to indications, selection criteria, contraindications, procedures, risks, and complications of desensitization for these reactions [86]. The definition of delayed used in this important paper might require further discussion.…”

“…Desensitization has been a reliable alternative to discontinuation of first-line drugs and different desensitization protocols have been developed [5,6,7,8,9,12,13,14,16,17]. Rapid desensitization was performed with rituximab because it was the most effective option for our patients with hematologic malignancies.…”
