Acantholytic dyskeratotic acanthoma is a rare variant of epidermal acanthoma. It has a flat, plaque‐like structure and is characterized microscopically by acantholysis and dyskeratosis. Eccrine syringofibroadenomatous hyperplasia is benign and likely reactive. It has recently been considered as a hyperplastic process affecting the eccrine ducts rather than the neoplasm because of its pathological heterogeneity and wide clinical associations. In this article, we present the case of 97‐year‐old Japanese women with a 10‐mm wide, painful acantholytic dyskeratotic acanthoma accompanied by syringofibroadenomatous hyperplasia in the right femoral region. Although syringofibroadenomatous hyperplasia is known to occur as a reactive process with various dermatoses and cutaneous tumors, to date, there have been no reports of cases of acantholytic dyskeratotic acanthoma accompanying syringofibroadenomatous hyperplasia. Moreover, this case also includes the unusual finding of an increase in the mature sebocytes in the area of the syringofibroadenomatous hyperplasia.

Introduction: Subungual acantholytic dyskeratotic acanthoma is a rare benign tumor of epidermal keratinocytes characterized by acantholysis and dyskeratosis. Only 7 other cases have been published in the literature. Case Presentation: A 29-year-old male presented with painful erythronychia and onycholysis of the left thumbnail. He worked as an electrician and was ambidextrous. Two months prior to the onset, he reported completing a mechanical task which led to repeated and sustained pressure to his left thumb. The clinical diagnosis was onychopapilloma, and a longitudinal excision was performed to remove the tumor. However, on histopathological examination, the primary features were hyperkeratosis, acantholysis, and dyskeratosis, consistent with a final diagnosis of subungual acantholytic dyskeratotic acanthoma. Conclusion: Nail trauma was reported by our case as a preceding feature, and this has not been described previously. Pain may accompany nail changes in the presentation of this tumor. Although subungual acantholytic dyskeratotic acanthoma is a benign tumor, it caused disruption in productivity, and our case highlights the need for timely access to specialist nail services.

Introduction: Focal acantholytic dyskeratosis is a distinctive histological pattern first described by Ackerman in 1972, consisting of focal suprabasal clefts in the epidermis and dyskeratotic cells at all levels of the epidermis with hyperkeratosis and parakeratosis. The first case of subungual acantholytic dyskeratosis acanthoma (ADA) was reported in 1990. This subungual variant is a very rare entity. Case Presentation: A 63-year-old female patient consulted for a longitudinal erythronychia (LE) associated with distal onycholysis. Dermoscopy showed LE with a few splinter hemorrhages, with localized hyperkeratosis and thinning of the nail plate at the free edge. Histopathology confirmed the diagnosis of ADA. Conclusion: ADA is a rare tumor that could mimic various inflammatory or neoplastic conditions. To our knowledge, this is the eighth reported case in the literature. This uncommon entity could mimic various inflammatory or neoplastic conditions.
