Dermoscopy of basosquamous carcinoma

Abstract: BSC appears to have overlapping dermoscopic features of BCC and invasive SCC, and detection of at least one dermoscopic criterion of both BCC and SCC should raise suspicion for the tumour. Appreciation of the dermoscopic patterns of BSC might assist in the timely and accurate diagnosis and subsequent optimal management of this unusual and potentially metastatic skin tumour.

“…Specifically, the list of dermoscopic structures of BSC includes unfocused arborizing vessels, ulceration and blue-gray blotches, but also keratin masses, white structureless areas, superficial scale, blood crusts and blood spots in keratin masses. Interestingly, at least one BCC and one SCC-related criterion was found in nearly all BSC included in the later study [25].…”

“…Dermoscopy was shown to mirror perfectly the peculiar histopathologic nature of BSC, since features related to BCC and features related to SCC were simultaneously present [25]. Specifically, the list of dermoscopic structures of BSC includes unfocused arborizing vessels, ulceration and blue-gray blotches, but also keratin masses, white structureless areas, superficial scale, blood crusts and blood spots in keratin masses.…”

“…Dermoscopic patterns may facilitate early diagnosis and accurate management, which comprises BCC-related criteria, as well as features that are known to characterize invasive SCC [9]. The most frequently detected criteria were: Unfocused (peripheral) arborizing vessels keratin masses, white structureless areas, superficial scale, ulceration or blood crusts, white structures, blue-grey blotches and blood spots in keratin masses [9,10]. BSC appears to have overlapping dermoscopic features of BCC and invasive SCC, and detection of at least one dermoscopic criterion of both BCC and SCC should raise suspicion for the tumor [10].…”

“…It is in the spectrum between BCC and SCC. It has more aggressive clinical course and higher risk of local recurrence and metastasis compared to BCC There is only one previous study about dermoscopic features of BSC (4). In that study, the most frequently detected dermoscopic criteria in 22 lesions were: unfocused peripheral arborizing vessels (73%), keratin masses (73%), white structureless areas (73%), superficial scale (68%), ulceration or blood crusts (68%), white structures (64%), bluegrey blotches (ovoid nests) (59%) and blood spots in keratin masses (55%).…”
