Dermatologists' Accuracy in Early Diagnosis of Melanoma of the Nail Matrix

Abstract: To measure and compare the accuracy of 4 different clinical methods in the diagnosis of melanoma in situ of the nail matrix among dermatologists with different levels of clinical experience. Design: Twelve cases of melanonychias (5 melanomas and 7 nonmelanomas) were presented following 4 successive steps: (1) clinical evaluation, (2) evaluation according to the ABCDEF rule, (3) dermoscopy of the nail plate, and (4) intraoperative dermoscopy. At each step, the dermatologists were asked to decide if the lesion w… Show more

“…Although melanocytic activation is the most common cause of LM in pigmented individuals who frequently have benign racial melanonychia involving multiple digits [2][3][4], melanocytic proliferation raises more concerns because LM may indicate an early stage of nail apparatus melanoma. The differential diagnosis to tell whether melanoma is the causative factor of LM remains an important task for dermatologists [5].…”

“…For a series of skin neoplasms, κ ranged roughly between 0.54 and 0.64 [11]. Interobserver agreement on clinical findings for in situ nail matrix melanoma was approximately 0.7 for experts and 0.62 for seniors [10]. Ginarte et al [12] evaluated dermatological clinical signs in a similar way as we did.…”

“…mSWAT) may be based, to our knowledge, this issue has not been addressed in previous studies. Surprisingly, a brief review of the literature suggests that interdermatologist agreement about the clinical diagnosis of neoplastic diseases usually does not exceed a moderate range [10,11], and it is even lower when clinical signs are compared [12]. In fact, additional techniques have been proposed in order to improve the accuracy of staging for skin lesions in MF [3,13,14].…”
