Der dexamethason-suppressions-test bei anorexia nervosa

Abstract: In patients suffering from anorexia nervosa (n = 20) a significant correlation was found between pathological DST results and weight loss. In this respect, patients with bulimic symptomatology do not differ from the main anorectic group. In addition, the correlation was unaltered by the length of the disease. These results are interpreted as indicative of secondary regulatory disturbances of the hypothalamic-hypophyseal-adrenal axis, whereas they are inconsistent with a close connection between anorexia nervos… Show more

“…All the three anorexics in the study were well established on refeeding programmes, and thus were gaining weight when seen. There are reports that cortisol hypersecretion in anorexics occurs in the presence of weight loss (Kuhs & Mester, 1985). Table 2 shows that, ofthe 3 anorecxics included in the study, the two who had their cortisols collected were not hypersecreting, and were therefore comparable with the mixed diagnostic group.…”

“…Moreover, the hyperactivity of the HPA axis differs in depression and AN, with a greater involvement of AVP in depressive disorder and perhaps more reliance on CRH to drive the axis in AN [98] . Some authors proposed malnutrition as the main determinant responsible for HPA axis alterations, because of the significant correlation between pathological DST results and the weight loss already mentioned [114][115][116][117] . Consistent with this interpretation is that the weight recovery is associated with a normalization of plasmatic and urinary cortisol levels [94,97] and with a significant decrease in the number of secretory bursts [97] .…”

“…They are in this respect inconsistent with a close noso logical connection concerning anorexia ner vosa [9,22], obsessive-compulsive syn dromes or endogenous depression, because a significant correlate was found between pathological DST results and loss of weight, psychic and somatic stress and/or distur bance of biological rhythms.…”
