Depression and Antidepressants During Pregnancy: Craniofacial Defects Due to Stem/Progenitor Cell Deregulation Mediated by Serotonin

Abstract: Depression is a common and debilitating mood disorder that increases in prevalence during pregnancy. Worldwide, 7 to 12% of pregnant women experience depression, in which the associated risk factors include socio-demographic, psychological, and socioeconomic variables. Maternal depression could have psychological, anatomical, and physiological consequences in the newborn. Depression has been related to a downregulation in serotonin levels in the brain. Accordingly, the most commonly prescribed pharmacotherapy … Show more

“…2,3 Serotonin, noradrenaline, and dopamine levels in synaptic cles are thought to be out of balance when depressive symptoms start to appear. 4,5 The rst medication initially approved for the on-demand treatment of men with PE (Premature Ejaculation) is dapoxetine hydrochloride (DPX), which belongs to the class of medicines that are called selective serotonin reuptake inhibitors. 6 The DPX causes a delay in ejaculation by blocking the serotonin (5-HT) transporter, which also increases serotonin's activity at the postsynaptic cle.…”

“…Lifetime risk of major depression among women approaches 25% and is highest during their childbearing years, resulting in an estimated 8% of pregnant women continuing antidepressant therapy throughout pregnancy. Concerns for the health outcomes of newborns in these pregnancies are mounting 1,2 . The once heavily prescribed SSRI, paroxetine (Paxil) has been re‐classified for pregnancy risk from class C to class D, indicating its potential risks to the developing foetus, specifically for cardiac anomalies 3 .…”

“…1,2 The once heavily prescribed SSRI, paroxetine (Paxil) has been re-classified for pregnancy risk from class C to class D, indicating its potential risks to the developing foetus, specifically for cardiac anomalies. 3 Though data correlating use of anti-depressant medications including the commonly prescribed class of selective serotonin re-uptake inhibitors (SSRI) to birth defects remain somewhat controversial, 2,4,5 there are now clinical and pre-clinical reports linking SSRI use and the developmental disruption of the craniofacial skeleton. 1,2 In mammals, multiple bones create the craniofacial skeleton.…”

“…In addition, the structure of the head is one of the most complex in the entire body. Different tissues with embriologic origins and complex signalling mechanisms seem to be involved during growth and development, 1 and malformations can originate from a variety of causes, including genetic factors, 2 infections, 3 pollutants 4 and prescription drugs 5 . Craniofacial malformations are not uncommon, 6 and their occurrence can cast severe burden on the quality of life of individuals, even after treatment 7 .…”
