Deployment of ACT antimalarials for treatment of malaria: challenges and opportunities

Abstract: Following a long period when the effectiveness of existing mono-therapies for antimalarials was steadily declining with no clear alternative, most malaria-endemic countries in Africa and Asia have adopted artemisinin combination therapy (ACT) as antimalarial drug policy. Several ACT drugs exist and others are in the pipeline. If properly targeted, they have the potential to reduce mortality from malaria substantially. The major challenge now is to get the drugs to the right people. Current evidence suggests th… Show more

“…To reduce malaria over diagnosis and misdiagnosis that occur in malaria endemic countries in Africa and Asia [11], the World Health Organization in 2010 recommended that diagnosis of malaria should be confirmed through parasite-based test; which could be Microscopy or Rapid Diagnostic Test (RDT) before commencement of treatment [10]. It is a good finding that 90.7% of the doctors working in the three key departments in the hospital that manage malaria cases were knowledgeable about this.…”

“…This is to minimize malaria over diagnosis and misdiagnosis observed in malaria endemic countries in Africa and Asia [11]. Microscopy is the gold standard for malaria diagnosis, but it has some drawbacks such as being dependent on trained and skilled personnel, availability of power supply, and availability of consumables required for microscopy [12].…”

“…(2) No evidence supported including diagnostic testing with a subsidy for antimalarials, although all agreed that increasing the use of diagnostic tests was another priority. 22 (3) No other practicable mechanisms in place of, or in addition to, a global subsidy would be better at reaching the poorest of the poor. Agreement was nearly unanimous that implementation of the AMFm should not be delayed, with continued recognition of the need for careful monitoring and operational research, which had been articulated originally by the IOM committee.…”

“…Alternative therapies such as artemisinin combinations for treatment of CQ-resistant malaria parasite infections have been developed. However, these therapies do not meet the simplicity of use and low cost of CQ (55).…”
