ABSTRAKKista dentigerus merupakan kista yang berhubungan dengan gigi yang tidak erupsi berasal dari sisa epitelial organ pembentuk gigi. Gejala klinis menunjukkan pembengkakan yang tumbuh lambat dan tidak sakit pada rahang. Gambaran radiografis memperlihatkan area radiolusen uniokular yang mengelilingi mahkota gigi yang tidak erupsi. Proses infeksi kronis pada kista dentigerus dapat menyebar lambat membentuk abses subperiosteal dan melewati barrier kulit membentuk fistel ekstra oral. Studi kasus ini laporkan seorang wanita 56 tahun yang datang ke poli Bedah Mulut dan Maksilofasial RSUP Dr Sardjito dengan kasus kista dentigerus pada regio angulus mandibula kanan dengan fistel ekstra oral pada pipi kanan. Penatalaksanaan kasus adalah pemberian antibiotik, enukleasi kista, pengambilan gigi 45, 47 dan 48 yang impaksi sertafistulektomi dengan pendekatan ekstra oral dan sinus shoe polishing technique di bawah bius umum. Evaluasi 3 minggu pasca operasi menunjukkan penyembuhan ekstra oral dan intra oral sempurna serta tidak terbentuk jaringan parut. MKGK. Kista ini juga disebut kista folikular karena dianggap berasal dari struktur mesodermal folikel gigi. Kista dentigerus disebabkan oleh gigi impaksi, gigi tambahan (supernumerary), gigi yang ektopik ataupun gigi yang terdapat pada rongga sinus. 1,2 Insidensi kista dentigerus sekitar 25 % dari seluruh kista rahang. Sering terjadi pada usia dekade kedua-ketiga dan laki-laki sedikit lebih banyak mengalami kista ini dibanding wanita. Sebagian besar kista dentigerus melibatkan molar tiga mandibula kemudian diikuti kaninus maksila. 3 Gejala klinis yang terlihat adanya pembengkakan yang tumbuh lambat, tidak sakit, palpasi keras menunjukkan adanya ekspansi kortikal. Jika ada rasa sakit dan pembengkakan yang tumbuh cepat mengindikasikan adanya peradangan. Jika kista ini diaspirasi akan diperoleh cairan jernih kekuningan. 3 Gambaran histologis dinding kista memperlihatkan bahwa kista dilapisi oleh reduced enamel epithelium. Stroma jaringan ikat menunjukkan gambaran tipe primitif ektomesenkim. Temuan tersebut tergantung ada tidaknya komponen inflamasi pada kista. Kista yang tidak terinfeksi memiliki lapisan epitelium yang padat 2-4 lapis tersusun oleh ektomesenkim primitif. Lapisan sel ini sedikit kuboid dibanding kolumnar dan terlihat rete peg. An intranasal tooth (INT) is an ectopic tooth erupting into the nasal cavity. It is a rare clinical entity. Ectopic and supernumerary teeth may be present in many regions of maxillofacial skeleton. Ectopic teeth may be supernumerary, deciduous or permanent. The clinical manifestations of intranasal tooth are quite variable and they may cause a variety of symptoms and complications. Their clinical and radiographic presentations are classical, posing little challenge to the diagnostician. The identification of such teeth can be important since they have potential to cause considerable morbidity. Here, we describe a case of intranasal tooth along with possible etiopathogenesis, clinical and radiographic features, differential diagnosis, potential complications and treatment modalities.

ABSTRACT:An ectopically erupted tooth with infected dentigerous cyst within the maxillary sinus of a child is a rare occurrence. A twelve year old male child presented with rhinorrhoea, haliotosis and swelling over the right cheek. It was diagnosed to be a large dentigerous cyst with ectopic tooth and right maxillary sinus.However it is a rare entity in children, commonly occur in mandible of middle aged persons. Early diagnosis and treatment is an essential as it has greater tendency to produce some resorption of other teeth as compare to other jaw cyst. Although marsupialization is treatment of choice in children but in the present case the cyst had lined the antral cavity hence enucleation had to be done through caldwell luc approach.
