Demonstration and identification of monoclonal proteins in the urine of patients with Sjogren's syndrome.

Abstract: SUMMARY Fresh sera and concentrated urine from 17 patients with primary Sjogren's syndrome (SS) were fractionated by high-resolution agarose electrophoresis to investigate the presence of monoclonal immunoglobulins or their components. Homogeneous protein bands were found in the gamma-globulin region in 47% of serum samples and 76% of urine specimens of all patients tested. These monoclonal proteins were detected more often in patients with extraglandular SS (77% in serum, 100% in the urine) than in patients w… Show more

“…In patients with mild disease, manifest by sicca symptoms alone, such hypergammaglobulinemia is usually polyclonal, but oligoclonal or monoclonal gammopathies appear in patients with more severe lymphoproliferation and extraglandular manifestations (Tzioufas et al, 1987). A large number of patients had monoclonal immunoglobulins in their serum or urine (Moutsopoulos et al, 1983(Moutsopoulos et al, , 1985 when testing was done with high-sensitivity assays. The incidence was more common in urine than in serum and increased with disease severity.…”

“…Analogous observa tions have been made by other investigators using sim ilar techniques in Burkitt's lymphoma [8]. Sjogren's syndrome [9][10][11], diffuse well-differentiated lympho cytic lymphoma [12] and lymphoplasmacytic lym phoma [13].…”

“…Recent studies have demonstrated the presence of a monoclonal K or A light chain in the sera and urine of all patients with extraglandular disease (21,22). This finding suggests that the monoclonal process may start very early in the process of this disease, along with the polyclonal B lymphocyte hyperreactivity .…”
