Background: Treatment data for latent tuberculosis infection (LTBI) among potential living kidney donors are scarce.Methods: This retrospective study was performed to evaluate the prevalence of positive QuantiFERON-TB Gold In-Tube (QFT-GIT) among potential living kidney donors that were screened from 2009 to 2017. We investigated if there was any difference in the time to donation between QFT-GIT-positive and QFT-GIT-negative donors.We assessed the regimens used to treat LTBI and whether the recipients of QFT-GIT-positive donors developed active tuberculosis (TB). Results: Forty out of 427 (9%) potential living kidney donors had a positive QFT-GIT. QFT-GIT-positive donors were as likely as negative donors to undergo donation (30 [75%] vs 315 [81%], P = .33). The time from QFT-GIT testing to donation was longer among QFT-GIT-positive donors (median 221 days [range: 4-1139] vs 86 days [range: 3-1887], P = .001). Twelve-week rifapentine (RPT)/Isoniazid (INH) was the most common treatment used and was not associated with significant adverse reactions. There was a trend toward longer time to donation among QFT-GIT-positive donors who were treated for LTBI compared with QFT-GIT-positive donors who were not (252 days [range: 88-1139] vs 95 days [range: 4-802], P = .05). Twenty-nine recipients of QFT-GIT-positive living kidney donors were evaluated. Eleven of these recipients received kidneys from donors that were not treated for LTBI. Two of these recipients were treated with INH post-transplantation. Conclusions: The time from QFT-GIT testing to donation was longer among QFT-GIT-positive donors. The short-course regimens appear to be excellent options for LTBI treatment among living kidney donors and avoid delaying organ donation further.

ÖzetAmaç Solid organ transplantasyonu yapılan hastalarda tüberküloz (TB) insidansı normal popülasyona göre daha yüksektir. Bu hastalarda TB önemli bir morbidite ve mortalite nedenidir. Transplant adaylarında aktif veya latent TB için tarama yapılması gereklidir. Bu çalışmada solid organ nakli öncesi donör ve alıcı adaylarında 'QuantiFERON TB Gold test' ('QFT-G')'i kullanılarak TB prevalansının ortaya konulması amaçlanmıştır. Materyal ve MetodÇalışma 01.07.2019-01.08.2019 tarihleri arasında solid organ nakli alıcısı ve vericisi ön tanısıyla organ nakli bölümlerine başvuran ve 'QFT-G' (In-Tube Method, Cellestis Limited, Australia) testi çalışılmış toplam 667 hasta çalışmaya dahil edilmiştir. Bulgular: Bu hastaların 227'si donör adayı, 440'ı alıcı adayıdır. Hastane bilgi yönetim sistemi üzerinden kayıtlar incelenmiş ve retrospektif olarak sonuçlar değerlendirilmiştir. Kategorik değişkenler açısından gruplar arasındaki karşılaştırmalar Ki Kare testi ile yapılmıştır.Bulgular Transplantasyon alıcı adaylarındaki genel 'QFT-G' TB poziti ik oranı %20,7; verici adaylarında ise %13,2 idi. Alıcı adaylarındaki 'QFT-G''i poziti ik oranı verici adaylarına göre anlamlı yüksek bulundu (p=0.018). Solid organ nakli alıcı adayları arasında 'QFT-G''i poziti ik oranı karaciğer alıcılarında %19,5, böbrek alıcılarında %21,8, kalp alıcılarında %25.0 idi ve poziti ik oranları açısından aralarında anlamlı fark yoktu (p=0.757).Sonuç Çalışmamız ülkemizden solid organ transplantasyonu donör ve alıcılarındaki ilk TB taramaları arasında olması nedeniyle önem taşımaktadır. Ayrıca hem aktif hem de latent TB'un taranmış olması çalışmamız verilerinin önemini arttırmaktadır. Çalışmamız solid organ transplantasyonu hem donör ve yeni organ kaynaklı TB, hem de alıcı kaynaklı latent enfeksiyonun da yakalanması gerekliliğini ortaya koymuştur. Çalışmamız verilerinin özellikle ülkemizde hem araştırmacılara hem de klinisyenlere nakil öncesi aktif veya latent TB insidansı ile ilgili önemli bilgi sağlayacağını düşünmekteyiz.

Abstract: Our high prevalence was likely a result of the high number of foreign-born RTC. Half of our small subset of low/intermediate-positive QFT-GIT patients reverted to negative. QFT-GIT-positive patients were more likely to have their LDKT delayed.
