Sesame allergy is a life-threatening disease that has been growing globally with poorly understood mechanisms. To protect sensitive consumers, sesame is regulated in many countries. There were four research goals for this work on sesame allergy: (i) to map the timeline, and the extent of its global rise; (ii) to dissect the complexity of the disease, and its mechanisms; (iii) to analyze the global regulation of sesame; and (iv) to map the directions for future research and regulation. We performed a literature search on PubMed and Google Scholar, using combinations of key words and analyzed the output. Regulatory information was obtained from the government agencies. Information relevant to the above goals was used to make interpretations. We found that: (i) the reports appeared first in 1950s, and then rapidly rose globally from 1990s; (ii) sesame contains protein and lipid allergens, a unique feature not found in other allergenic foods; (iii) it is linked to five types of diseases with understudied mechanisms; and (iv) it is a regulated allergen in 32 advanced countries excluding the USA. We also provide directions for filling gaps in the research and identify implications of possible regulation of sesame in the USA.

Background and AimThe prevalence of eosinophilic esophagitis (EoE) is rising in the West. However, data from the Indian subcontinent is limited. In this prospective cross‐sectional study, we estimated the prevalence of EoE among children undergoing elective upper gastrointestinal endoscopy (UGIE).MethodsWe enrolled 200 consecutive children (123 boys, median age 10.25 years [interquartile range 8.25–14.5]) between March 2020 and November 2022 at our center. Clinical characteristics, endoscopic findings, and laboratory parameters were noted. A total of 12 mucosal biopsies (3 each from the middle and lower third of the esophagus, stomach, and duodenum) were obtained. EoE was diagnosed if the peak eosinophil count was ≥15/high‐power field (HPF) in absence of gastric and duodenal eosinophilia.ResultsThe commonest indications for UGIE were gastroesophageal reflux disease‐like symptoms (29%), inflammatory bowel disease (22.5%), celiac disease (15%), and abdominal pain (13%). EoE was detected in seven children, suggesting an overall prevalence of 3.5%. Of the 20 children evaluated for dysphagia, 4 (20%) had EoE. Also, two of three (67%) children presented with food bolus impaction along with dysphagia had EoE. Of the seven children with EoE, three (43%) had bronchial asthma, two (28.5%) had peripheral eosinophilia, and one (14%) had elevated serum IgE. Trachealization and linear furrows were found in 57% and 71% cases, respectively. Four children received high‐dose proton pump inhibitor (PPI) for 12 weeks, two received PPI+ stricture dilatation, and one received systemic steroids. All achieved clinical, endoscopic, and histopathological remission.ConclusionHospital‐based prevalence of EoE among children undergoing elective UGIE was 3.5%. EoE patients had favorable outcomes with PPI.

Eosinophilic Esophagitis (EoE) is a chronic condition characterized by eosinophilic infiltration of the esophageal mucosa and symptoms resulting from esophageal dysfunction. The most important symptom is dysphagia, which causes an impaired quality of life and significant healthcare costs. Food allergies have a key role in the development of EoE: the removal of specific foods from the diet is sufficient to reduce esophageal inflammation and improve symptoms. The diagnosis of EoE is based on a combination of symptoms, eosinophilic infiltration and the absence of other conditions which can explain esophageal eosinophilia (mainly gastro-esophageal reflux disease). Diagnostic delay should be avoided because if left untreated, a fibrostenotic phenotype can develop with strictures. Current therapeutic approaches are based on the “3 D’s” concept: Dietary management, Drug therapy and esophageal Dilation. Dietary management can be based on an elemental diet (amino acid-based), which is rarely used in adults because of the poor palatability; an empiric elimination diet (2- 4-6 food elimination diet). Medical treatment is still the most commonly used approach in EoE. In particular, swallowed topic corticosteroids such as topical budesonide or fluticasone have been shown to be very effective in reducing esophageal inflammation. More recently, biological treatments have been evaluated as novel treatment options for EoE, targeting different cytokines or receptors in the Th2 immune reaction. Finally, in case of esophageal stricture, dilation is warranted.
