Demographic and clinical characteristics of primary glomerular diseases in Turkey

Abstract: The presented study displayed important data about the epidemiology of primary glomerular diseases among adults in our country. The predominance of membranous nephropathy in contrast to other countries, in which the most frequent etiology is IgA nephropathy, seems to be due to differences in the indications for renal biopsy.

“…Notably, 23.8% of elderly and 24.8% of younger patients with pauci-immune GN suffered from nephrotic-range proteinuria. Pauci-immune GN being one of the leading diagnoses made upon kidney biopsy among elderly individuals was reported by others [13, 25, 28, 29]. The peak frequency and the highest relative frequency of this disease in our patients studied occurred at the age of 55-64 years (Fig.…”

“…In a vast majority of cases pauci immune GN is associated with the presence of ANCA vasculitis, with peak incidence of 52.9 per million being observed among people aged 65–74 [30]. It is worth mentioning that in contrast to younger individuals, in the elderly vasculitis more commonly manifests with renal disease, and less frequently involves upper respiratory tract [29]. Vasculitis-related symptoms such as myalgia and headache may be easily misdiagnosed as common afflictions of the elderly, e.g., polymyalgia rheumatica [31].…”

“…According to some other reports, male patients were also bioptied more frequently than female (Romanian data-51.5%; Clinical Center Serbia-51.2%; Pisa, Italy-59%; Czech data-57.9%; Turkish data-55%). The mean age at the moment of renal biopsy was almost one decade higher in our patients than in that reported by others (two Romanian Centers-38.5±15.2; Clinical Center Serbia-39.1±13.8 years, Turkey-40.8±14.6 years) (5)(6)(7)(8)(9). This difference can be explained by different attitudes regarding the biopsy of the elderly.…”

“…As indicated in Figure , the proportion of MN increased significantly year after year. An increase in the frequency of MN has also been reported from other centres in China and other countries . Some authors have argued that the increase in MN incidence is due to an improvement in life expectancy .…”

“…A limited number of single centre studies have shown that the renal histopathological spectrum of proliferative glomerulonephritis (PGN) has continuously changed in recent years. One of these changes is the frequency of MN (15.7–29.2%), which has increased significantly . IgAN is still the most common PGD in China and other countries .…”
