Dementia With Lewy Bodies

Abstract: Dementia with Lewy bodies is an often missed diagnosis. Symptoms are often attributed to other disorders. A high clinical suspicion is helpful in accurate diagnosis, and presence of any of the core symptoms should initiate clinical suspicion of DLB. Distinguishing DLB from other disorders has important treatment implications.

“…The most common types of dementia are AD-related dementia (approximately 50–80% of all dementia cases) (Qiu et al, 2009 ; Abbott, 2011 ), vascular dementia (approximately 20–30%) (Abbott, 2011 ; Iadecola, 2013 ), dementia with Lewy bodies (between 15 and 35% according to Zupancic et al, 2011 , or less than 5% according to Abbott, 2011 ) and the frontotemporal dementia (FTD), which is the fourth most frequent form of presenile dementia (between 5 and 10%) (Abbott, 2011 ; Table 1 ).…”

“…Vascular dementia is a heterogeneous group of brain disorders associated with a cognitive impairment that is attributed to multifactorial cerebrovascular pathologies, such as hypoperfusion, oxidative stress, and inflammation (Iadecola, 2013 ). Dementia with Lewy bodies is characterized by the presence of α-synuclein aggregates in neurons and glial cells (Zupancic et al, 2011 ), and also associated with cholinergic as well as glutamate transmission deficiencies (Zupancic et al, 2011 ). It may also be concluded, that there is a great deal of overlap between the symptoms of different types of dementia (Abbott, 2011 ).…”

“…Lewy body dementias, the third most common type of dementia, results from the excessive accumulation of alpha‐synuclein protein, called Lewy bodies, in neurons. 34 LBD leads to cognitive impairments with recurrent visual hallucinations, lethargy, and a decrease in attention, as well as parkinsonism. 34 , 35 …”

“…DLB is considered to be the second most common form of neurodegenerative disorders after AD (Geser et al, 2005;Zupancic et al, 2011), affecting as many as 100,000 people in the UK and up to 1.3 million in the USA. Prevalence estimates range from 0% to 5% in the general population, and up to 30.5% of all dementia cases (Zaccai et al, 2005), and these are largely influenced by the clinical criteria used for DLB diagnosis.…”
