Delivery in Breech Presentation: The Decision Making

Abstract: In view of insignificant difference in the fetomaternal outcome balanced decision about mode of delivery on a case by case basis will go a long way in improving both foetal and maternal outcome. “…This is possibly because of the fact that in Indian women generally family life is completed by this age group due to early marriage and early age at first pregnancy (Table 2). 4,6 As evident from Table 3, present study correlates with previous studies by other authors, it favors incidence of extended (frank) breech more than complete breech with a rate of 68% and 32% respectively. [6][7][8][9] This is most probably because of a favorable engaging diameter in extended breech (bistrochanteric) and less space occupied by the narrow lower pole.…”

[6][7][8][9] This is most probably because of a favorable engaging diameter in extended breech (bistrochanteric) and less space occupied by the narrow lower pole. A positive correlation was observed with previous studies by other authors, that in majority of the studies, caesarean section was more common as a mode of delivery than vaginal route, in breech presentation; but the incidence should not be that low as observed in a few studies depicted in Table 4.…”

“…As shown in Figure 4, oligohydramnios and gestational hypertension were associated with breech in majority of cases undergoing caesarean section against study of Sonali et al, where post LSCS, bad obstetric history, gestational hypertension, oligos and fetal growth restriction were the indications for caesarean section. Table 3 shows that maternal morbidity was 10% in caesarean section against 3.4% in caesarean section and 4.2% in vaginal delivery in a study by Abha et al 3 From Table 4, term breech was seen in 88% of cases against 77.3% in the study by Abha Singh et al 3 56% had birth weight of 2.6-3kg and 14% had 3.1-3.5kg birth weight …”
