Delayed Treatment of Pulmonary Blastomycosis Causing Vertebral Osteomyelitis, Paraspinal Abscess, and Spinal Cord Compression

Abstract: A 36-year-old woman with gallbladder disease had an incidental finding of asymptomatic cavitary lung infection with Blastomyces dermatitidis. No treatment was given initially, and 2 months later she presented with vertebral osteomyelitis, paraspinal abscess, and spinal cord compression due to dissemination of the fungus. “…Itraconazole is an oral triazole which has excellent in vitro and in vivo activity against Blastomyces dermatitides. It has an efficacy rate of 90% at doses of 200-400 mg daily and the patient needs to be treated for 6 months [18]. Itraconazole has superior efficacy and lower side effects, so it should be used as the drug of choice in immunocompetent patients with blastomycosis.…”
