Delayed Successful Surgical Repair of Penile Fracture: A Case Report

Abstract: Penile fracture is a very rare urological emergency resulting from traumatic rupture of the tunica albuginea of one or both corpora cavernosa, usually during sexual intercourse. Immediate surgical treatment is the current standard of care with lower risks of late complications, including erectile dysfunction, penile curvature, and tunical scar formation. We, hereby, report an over delayed presentation (23 days) of a penile fracture, which was successfully managed surgically. Our case emphasizes on the fact tha… Show more

“…Penile fracture is an uncommon or, probably, a rarely reported urological emergency [1] commonly associated with extramarital affairs, sexual intercourse in out-of-the-ordinary locations, penile manipulation and unusual sexual positions(woman on top and "doggy style") [2,3]. It is a result of traumatic rupture of the tunica albuginea of one or both corpora cavernosa, due to twisting or bending of the penile shaft during erection] [4].…”

“…(It is able to withstand rupture at pressures above 1500 mmHg.) The tunica albuginea measures 2 mm in a flaccid penis and becomes thinner (0.25-0.5 mm) during erection [9]. Rupture of the tunica albuginea is more likely when the penis erect because the tissue of the tunica albuginea is thinner and vulnerable to a sudden increase in the intracorporeal pressure.…”

“…The etiology of penile fracture varies with the geographical area. In the Western hemisphere, sex is the majority cause of penile fracture cases [9,14]. In Japan, it was reported that only 19% of penile fracture cases resulted from sexual intercourse, with a larger proportion caused by masturbation and rolling in bed when the penis was erect [15].…”
