Delayed hypersensitivity to antiepileptic drugs in children

Abstract: Background: Antiepileptic drugs (AEDs) are widely used for the treatment of epilepsy, but they can be associated with the development of mainly delayed/non-immediate hypersensitivity reactions (HRs). Although these reactions are usually cutaneous, self-limited, and spontaneously resolve within days after drug discontinuation, sometime HR reactions to AEDs can be severe and life-threatening. Aim: This paper seeks to show examples on practical management of AED HRs in children starting from a review of what it i… Show more

“…Regarding clinical manifestations, sensitivity and specificity are higher in MPE, FDE, AGEP and DRESS than in SJS/TEN [ 90 , 96 ]. Interestingly, lymphocyte transformation test results have shown a good correlation with the algorithm of drug causality for the epidermal necrolysis (ALDEN) score, suggesting its usefulness for diagnosing severe reactions [ 97 , 98 ]. Moreover, its use to evaluate drug cross-reactivity [ 99 ] and to confirm the culprit drug in SJS/TEN cases with multiple drug therapy should be highlighted [ 98 ].…”

“…Individuals with specific HLA haplotypes are predisposed to develop DRESS. The most common implicated drugs include antiepileptics, allopurinol, sulfonamides, BL and dapsone [5 ▪▪ ,6 ▪▪ ,8,24–26]. The pathophysiology is thought to be a T-cell-mediated hypersensitivity reaction type IVb (Coombs and Gell classification) with increased IL-5 expression and increased eosinophil number and their activation and recruitment to the affected tissues [3 ▪▪ ,8].…”
