Delayed Admission and Management of Pediatric Acute Kidney Injury and Multiple Organ Dysfunction Syndrome in Children with Multiple Wasp Stings: A Case Series

Abstract: Multiple wasp stings may cause fatal complications, such as anaphylactic reactions, intravascular hemolysis, rhabdomyolysis, acute kidney injury (AKI), increased levels of liver enzymes, clotting abnormalities, or even death. AKI-related mortality due to multiple wasp stings may reach 25%, occurring within the early onset of disease; therefore, renal function should be continuously monitored within the first few days following the stings. Herein, we report 2 cases of AKI due to multiple stings of wasp (Vespa a… Show more

“…e pediatric dialysis unit has not been widely available in Indonesia [4]; therefore, referral system has been a major issue, since Indonesia is an archipelago consisting of 17,504 islands [23]. As a result of the complex referral system between islands, most patients have significant clinical deterioration upon admission [24]. is baseline hyperferritinemia is a distinctive challenge at our unit because providing patients with high ferritin levels with IV iron maintenance is controversial.…”

“…It would be therefore be advisable for such patients to be followed in nephrology clinic after their discharge. However, this result may also be related to the short hospitalization time of our patients (average 11 days), because, according to Ambarsari's report, the cure of acute kidney injury after wasp sting takes 3-6 weeks [41]. Our research also has limitations.…”

“…A key reason for this may be that Indonesia is the largest archipelago country in the world, comprising more than 17 000 islands with a population of over 260 million people that is predicted to grow by 28% in the next 25 years 14,15 . The complicated referral system in this setting has resulted in only complex cases with multiple complications being admitted to tertiary referral centers 16 . As of 2019, only two hospitals in the nation offered pediatric kidney transplantation (CMH and Dr Kariadi Hospital).…”
