Delay of Adjuvant Chemotherapy After Elective Mastectomy and Immediate Reconstruction in Breast-conservation Candidates

Abstract: The use of M-IR in breast-conservation candidates independently predicts for delay in initiation of adjuvant chemotherapy. Further study is needed to qualify the causes and clinical significance of these delays.

“…21 Multiple factors, including age, disease-specific characteristics, type of surgical procedure, and additional testing have been found to influence this trend. [21][22][23][24][25][26][27] In an era of increasing use of Oncotype DX to assist with chemotherapy treatment decisions, an aging population with comorbidities needing to be addressed, and the more prevalent use of MIR, it is not surprising that wait times to adjuvant therapy continue to increase.…”

“…Patients undergoing mastectomy with immediate reconstruction were more likely to experience a significant delay in the initiation of chemotherapy (>90 days; 7 patients [20.0%] vs 1 patient [2.9%]; p < 0.001). 25 It is not clear that this is in fact a clinically significant "delay" because it falls within the large window of acceptable timing for treatment. In addition, the matching in this case might also be imperfect for numerous reasons.…”
