In Germany, the number of reported syphilis cases increased between 11% and 22% per year between 2010 and 2014. We analysed syphilis surveillance data and data of four behavioural surveys on men who have sex with men (MSM) in Germany (2003, 2007, 2010, 2013) to assess if this rise is ongoing and to find possible explanations for it. Syphilis notifications increased in 2015 by 19% to a total of 6,834. This was mainly due to increasing notifications in MSM of all age groups in larger German cities. Data from the behavioural surveys on MSM in Germany showed a simultaneous increase of selective condom use as HIV-status-bases risk management strategy and the number of syphilis cases. MSM diagnosed with HIV reported condomless anal intercourse with non-steady partners more frequent than MSM not diagnosed with HIV or untested for HIV, but the latter also reported higher frequencies of this behaviour in the more recent surveys. Transmission in HIV-positive MSM probably plays an important, but not exclusive role, for the syphilis dynamics in Germany. A risk adapted routine screening for sexually active MSM and potentially innovative approaches to increase early screening and treatment of syphilis such as internet counselling, home sampling, home testing and broadening venue-based (rapid) testing, should be critically evaluated to effectively reduce syphilis infections.

Zusammenfassung Hintergrund Die Wirksamkeit der Benachrichtigung von Sexualpartner*innen über die Diagnose von sexuell übertragbaren Infektionen (STI), um Übertragungsketten zu unterbrechen, ist durch den internationalen Forschungsstand gut belegt. Verschiedene empirische Arbeiten verweisen auf die Bedeutung struktureller und individueller Faktoren für die Bereitschaft, Partner*innen zu benachrichtigen. Methoden Über quantitative und qualitative Befragungen von Nutzer*innen anonymer Teststellen konnten erstmals belastbare Daten dazu für Deutschland gewonnen werden. Im Erhebungszeitraum (Juli bis September 2019) wurden bundesweit insgesamt 845 Nutzer*innen anonymer Teststellen befragt: 721 füllten einen schriftlichen Fragebogen aus, mit 169 Befragungsteilnehmenden konnte zusätzlich ein etwa 30-minütiges leitfadengestütztes Interview realisiert werden. Ergebnisse Wie unsere Erhebungen zeigen, wurde etwa ein Drittel aller befragten Klient*innen bisher schon mindestens einmal von einem Sexualkontakt informiert, dass die Möglichkeit einer STI besteht. Generell wären die Klient*innen selbst dankbar über eine Benachrichtigung – am liebsten durch den infizierten Sexualkontakt persönlich. Das Thema Partner*innenbenachrichtigung (PB) bewegt die Nutzer*innen, viele empfinden Verantwortung für ihre Sexualpartner*innen und fast alle wollen sie informieren. Doch fällt es vielen schwer, ihre Sexualkontakte auf eine mögliche Infektion hinzuweisen. Deshalb wünschen sich vier Fünftel der 721 befragten Klient*innen, zu Möglichkeiten von PB beraten zu werden. Schlussfolgerung Klient*innen wünschen sich Informationsmaterial (analog und digital) sowie Beratung und Hilfen zur Gesprächsführung bzgl. PB (wie, wann und wo Sexualkontakte informieren?) und schließlich tatsächlich verfügbare Test- und Behandlungsangebote.

Little is known about the sexual health of male providers and clients of transactional sex. The data of participants of the European MSM Internet Survey (EMIS-DE; N = 50,086) who live in Germany were analysed. The outcomes were testing for and diagnoses of HIV and other sexually transmitted infections (STIs), partner numbers, illicit drug use, and sexual happiness in two populations: (1) men who offered and (2) men who paid for transactional sex in the past 12 months.In the age group of under 30 years (n = 19,138), providers of sexual services (escorts) were compared with non-providers; in the age group of 30 years and above (n = 30,948), we compared men who paid for sexual services with those who did not. We applied univariable and multinomial, multivariable logistic regression analyses in both age groups.Of those under the age of 30, 8% (n = 1529) had been paid for sex in the last 12 months, of which 49% were only paid once or twice. Escorts had lower educational achievements, lived more commonly in large cities, were more often born abroad, self-defined less frequently as gay, and were more frequently single. They reported higher numbers of sex partners, more drug use, and more sexual happiness. Escorts were more frequently tested for HIV and STIs, and among the tested, more diagnoses of HIV and STIs were reported, but those with HIV were less likely to be treated.Among those over 30 years, 11% (n = 3334) had paid for sex, the majority (58%) only once or twice. Payers were older, lived more commonly in large cities, and were more likely to be single, bisexual, or living with a woman. Clients used more illicit drugs, sexual enhancement drugs, and sedatives, but reported less sexual happiness. They were less likely to be tested for HIV and STIs.
